洛铂联合香菇多糖序贯性胸腔灌注治疗难治性恶性胸腔积液

目的：观察洛铂联合香菇多糖胸腔灌注治疗难治性恶性胸腔积液的疗效。方法入组治疗后复发的难治性恶性胸腔积液患者46例，分为2组。观察组22例治疗方案：胸腔置管后排净胸腔积液，将洛铂30 mg 溶解于生理盐水50 mL 中进行胸腔灌注，1周后灌注香菇多糖注射液4 mg ﹢生理盐水20 mL。对照组24例治疗方案：每周1次于胸腔灌注香菇多糖注射液4 mg ﹢生理盐水20 mL，直至胸腔积液完全引出，拔除引流管。2组治疗后评价疗效，同时观察毒副反应及对患者生活质量的影响。结果观察组总有效率为68.2%，生活质量明显改善率63.6%，均高于对照组的50.0%、37.5%（P 均﹤0.05）。2组均未出现由于严重毒副反应而不得不终止治疗病例。结论洛铂联合香菇多糖序贯性胸腔灌注治疗难治性胸腔积液疗效肯定，毒副反应轻，值得临床推广。

Lobaplatin Combined with Lentinan in the Treatment of Malignant Pleural Effusion

Objective To observe the clinical effect of lobaplatin combined with lentinan in the treatment of ma-lignant pleural effusion. Methods Forty-six patients with malignant pleural effusion were randomly divided into the control group(24 patients)and the trial group(22 patients). All the patients were treated with thoracentesis and inserted central venous catheter to drain pleural effusion. After exhaustion of pleural effusion,the control group was treated with lentinan by pleural perfusion,and the trial group was treated with lobaplatin on the base of the control group. The effect,toxicities and quality of life were compared of the two groups. Results In the trial group,the re-sponse rate was 68. 2% ,the improvement incidence was 63. 6% ,and were 50. 0% and 37. 5% in the control group(P ﹤ 0. 05). No withholding treatment caused by serious toxicities were observed in the two groups. Conclu-sion Lobaplatin combined with lentinan is safe and effective in the treatment of malignant pleural effusion.
