T1期乳腺癌行改良性保乳保腋窝手术与传统保乳手术的对比研究

[目的]比较T1期乳腺癌行改良性保乳保腋窝手术与传统保乳手术的治疗效果.[方法]回顾性分析2009年1月至2014年3月本院收治的行保乳手术的T1期乳腺癌患者102例,在知情同意的原则下,由患者自主选择治疗方案,按手术方式分为改良性保乳保腋窝手术组(A组,66例)和传统保乳手术组(B组,36例),术后均根据具体情况采用个体化的综合治疗.术后随访14~64个月.对两组患者的手术指标、手术并发症、术后局部复发率及乳房美学效果等相关指标进行对比分析.[结果]A组在手术时间、术中出血量、手术并发症发生率以及乳房美学效果上均优于B组,差异有显著性(P<0.05).A组与B组的局部复发率相近,差异无统计学意义(P>0.05).[结论]T1期乳腺癌行改良性保乳保腋窝手术治疗效果好,创伤小,值得临床推广应用.

Comparative Study of Modified Breast and Axillary Reservation Operation and Conventional Breast-Conserving Operation on T1 Stage Breast Cancer

[Objective]To compare the effect of modified breast and axillary reservation operation and con-ventional breast conserving operation on stage T1 breast cancer.[Method]Retrospective analysis of 102 pa-tients with stage T1 breast cancer who underwent breast conserving operation form January 2009 to March 2014 in our hospital.Under the principle of informed consent,patients chose their treatment programs.Ac-cording to the operation mode,they were divided into the modified breast and axillary reservation operation (Group A,n=66)and conventional breast conserving operation group (Group B,n =36).All the patients were treated with individual comprehensive therapy according to the specific situation,and were followed up for 14~64 months.The operation index,operation complications,rate of local recurrence and breast aesthetic effects of both groups were compared and analyzed.[Result]Group A in the operation time,amount of bleeding during the operation,operation complication rate and the effects on aesthetics of breast is superior to the Group B,the difference was significant (P <0.05).The local recurrence rate in the two groups are similar, there was no significant difference (P >0.05).[Conclusion]Modified breast and axillary reservation operation in patients with stage T1 breast cancer,the treatment effect is good,the trauma is small,it is worthy of clini-cal application.
