莫西沙星对多重耐药肺结核患者血清PCT和sTREM-1水平的影响

摘    要:目的]分析莫西沙星干预治疗对多重耐药肺结核(MDR-TB)患者血清降钙素原(PCT)和可溶性髓系细胞触发受体-1(sTREM-1)水平的影响.方法]连续性纳入140例MDR-TB患者,随机等分为莫西沙星组(n=70)和对照组(n=70),动态观察治疗后2个月和6个月血清PCT和sTREM-1水平改变,以及临床治愈情况.结果]治疗6个月后,观察组有效率为85.7%(60/70)显著高于对照组的65.7%(46/70),差异具有统计学意义(x 2=5.962,P=0.015<0.05).观察组在治疗后血清sTREM-1和PCT水平明显下降,血清sTREM-1在2个月和6个月两个时间点水平明显低于对照组患者,差异均具有统计学意义(均P<0.05),而血清PCT水平在治疗6个月后明显低于对照组患者,差异具有统计学意义(P<0.05).莫西沙星组患者无论是有效亚组还是无效亚组,血清PCT和sTREM-1水平在治疗后6个月与治疗前比较,差异均有统计学意义(P<0.05).而对照组患者,治疗后PCT并没有明下降趋势,而sTREM-1水平虽然有所下降,但并无明显统计学差异(P>0.05).结论]莫西沙星可以有效降低MRD-TB患者血清PCT和sTREM-1水平,可提高其治疗有效率.

Influence of Moxifloxacin on Serum Levels of PCT and sTREM-1 in Patients with Multiple Drug Resistance Tuberculosis
Abstract:Objective]To evaluate the effect of moxifloxacin intervention therapy on serum PCT and sTREM-1 in patients with multiple drug resistance tuberculosis(MDR-TB).Methods]One hundred and forty patients with MDR-TB were enrolled in this study and randomly divided into two groups:the moxifloxacin group(n=70)and the control group.Dynamic changes of serum PCT and sTREM-1 levels and the clinical efficacy were observed in 2 and 6 months after treatment.Results]After 6 months of treatment,the effective rate of the observation group was 85.7 % (60/70),which was significantly higher than that of the control group 65.7% (46/70),and the difference was statistically significant (x2 =5.962,P =0.015<0.05).The levels of serum sTREM-1 and PCT were significantly decreased in the observation group after treatment,the serum sTREM-1 level was significantly lower than that of the control group at 2 and 6 months,and the difference was statistically significant (mean P <0.05);the serum PCT level was significantly lower than that the control group after 6 months of treatment,the difference was statistically significant (P <0.05).The differences of the serum PCT and sTREM-1 levels of Moxifloxacin group(both in the effective subgroup or ineffective subgroup) were statistically significant 6 months before or after treatment (P <0.05);In the control group,PCT did not show a clear downward trend after treatment,while the level of sTREM-1 was decreased,but there was no significant difference(P >0.05).Conclusion]Moxifloxacin can effectively reduce serum PCT and sTREM-1 levels in patients with MRD-TB and can improve the treatment efficiency.
