真皮下血管网皮瓣在手部创伤修复中的应用

目的总结应用带蒂随意、轴型真皮下血管网皮瓣和吻合血管的真皮下血管网皮瓣修复手部软组织缺损65例72个皮瓣的临床效果。方法应用带蒂随意和轴型真皮下血管网皮瓣修复手部软组织缺损53例59个皮瓣，皮瓣长宽比例最大3．5：1，断蒂时间4-20d；吻合血管的真皮下血管网12例13个皮瓣，修复面积最大为60％。结果除1例随意皮瓣远端有1cm×3cm皮肤坏死外，其余皮瓣全部成活，均获得满意疗效。结论真皮下血管网皮瓣修复手部软组织缺损效果优于传统皮瓣。

Application of flap with subcutaneous capillary network in repairing soft tissue defects in hand traumas

Objective To summarize the clinic effect and experience of random,axial pedicle flap or vascular anastomosis flap all with subcutaneous capillary network in repairing soft tissue defects in hand in 65 patients 72 flaps.Methods 59 flaps in 53 patients were repaired with random,axial pedicle flaps with subcutaneous capillary network.The maximum ratio of length to width was 3.5：1. The shortest detachment time was 4 repaired with vascular anastomosis days, flaps while the longest 20 days. 13 flaps in 12 patients were with subcutaneous capillary network. The maximum repairing area was 60%.Results All flaps survived completely with satisfactory effect except 1 case with distal skin necrosis.Conclusion Flap with subcutaneous capillary network is superior to conventional flap in repairing soft tissue defects in hand traumas.
