冠心病患者血浆Lp-PLA_2浓度变化及与左心功能的关系

目的研究冠心病患者血浆Lp-PLA2水平变化,探讨Lp-PLA2与左心功能的相关性及与AS发生发展的关系。方法收集急性心肌梗死患者30例,不稳定型心绞痛患者32例,稳定型心绞痛患者28例和对照组27例。用ELISA法和超声心动图检测血浆Lp-PLA2浓度及左室收缩功能指标。结果①AMI组和UAP组Lp-PLA2浓度明显高于SAP组和对照组(P<0.01),SAP组亦高于对照组(P<0.05);②血浆Lp-PLA2水平与LVEF呈负相关(r=-0.693,P<0.001),与LVEDV、LVESV呈显著正相关(r=0.509,P<0.001;r=0.658,P<0.001)。结论 Lp-PLA2与冠状动脉粥样硬化斑块稳定性有关,在AS的发生、发展中起重要作用,且与心脏收缩功能显著相关,在冠心病患者诊断上可能有指导意义。

The Change of the Plasma Concentration of Lp - PLA2 in Coronary Heart Disease Patients and its Relationship with the Left Cardiac Function

Objective By studying the changes of Lp-PLA2 concentration in plasma in patients with coronary heart disease to investigate the correlation between Lp-PLAz and left cardiac function and AS. Methods Collecting 30 patients with acute myocardial infarction group, 32 patients with unstable angina group, 28 patients with stable angina pectoris group and 27 patients with control group. Using ELISA method and echocardiography to detect plasma concentration of Lp-PLA2 and left ventricular systolic function indexes. Result （1）The Lp-PLA2 in AMI group, UAP group are higher than SAP group and control group（P〈0.01）, SAP group are also higher than the control group（P〈0.05）; （2）The level of Lp-PLA2 in plasma is a significantly positively correlated with LVEF（r=-0.693, P〈0.001）, A significant positive correlated with LVEDV and LVESV（r=0.509, P〈0.001; r=0.658, P〈0.001）. Conclusion Lp-PLA2 is correlation to the stability of coronary artery atherosclerotic plaque, play an important role in the appearance and development process of AS. The level of Lp-PLA2 in plasma and cardiac function index have obvious correlation, it may be useful as a guide in coronary artery disease of diagnosis.
