老年人大肠癌肠梗阻的急诊处理（附48例报告）

目的　总结老年人大肠癌肠梗阻急诊手术处理的体会。方法　 1985～ 1999年间收治老年人大肠癌肠梗阻 4 8例 ,分析其发病情况、术前诊断和急诊处理。结果　老年人大肠癌梗阻就诊时间晚 ,术前能定性定位确诊的机率低 ;右半结肠癌梗阻均作I期切除吻合术 ,无手术并发症 ,左半结肠癌视具体情况而定。结论　老年人大肠癌肠梗阻就诊时间晚 ,术前B型超声或CT检查对初步明确梗阻部位及病因有一定帮助。年龄不是手术危险的主要因素。术中应视具体情况合理选择术式。

Emergency treatment of 48 elderly patients with colorectal cancer ileus

Objective To summarize the experience of emergency operative treatment of 48 elderly patients with colorectal cancer ileus. Methods Onset, preoperative diagnosis and emergency treatment of 48 elderly patients with colorectal cancer ileus were retrospectively analyzed from 1985 to 1999. Results The visiting time of elderly patients with colorectal cancer ileus was late. The probability of preoperative qualitative and local diagnosis was low. The right carcinoma of colon all received primary resection and anastomosis. No operation-related complications occurred. Left carcinoma of colon was treated on the basis of general conditions.Conclusions The visiting time of elderly pateints with colorectal cancer ileus was late. Preoperative B ultrasound or CT examinations was helpful for diagnosing obstructed location and causes. Age was not the key factors for surgery.
