欧堡全景在糖尿病视网膜病变筛查中的作用

摘    要:目的 探讨欧堡全景眼底照相作为筛查糖尿病视网膜病变方法的可行性.方法 回顾性分析经糖尿病中心确诊为糖尿病并参加筛查患者的临床资料,共75例患者,141只眼.所有病例均进行欧堡全景眼底照相和七方位散瞳眼底照相,比较欧堡全景眼底照相筛查糖尿病视网膜病变的敏感度、特异性,以及与七方位眼底照相检查的一致性.以黄斑出血和渗出性病变为指标评价欧堡全景眼底照相筛查糖尿病黄斑病变的敏感度、特异性.结果 以轻度非增殖性糖尿病视网膜病变为阈值,欧堡全景眼底照相检查的敏感度为73.45%,特异性为75.01%,一致性为0.625.以中度非增殖性糖尿病视网膜病变为阈值,欧堡全景眼底照相的敏感度为94.85%,特异性为98.82%,一致性为0.912.欧堡全景眼底照相筛查黄斑区出血和黄斑渗出性病变的敏感度和特异性分别为78.2%、87.5%和75.4%、86.2%.结论 欧堡全景眼底照相可成为一种非移动免散瞳糖尿病视网膜病变筛查方法.

Feasibility on Screening of Diabetic Retinopathy by Optomap Panoramic 200

Abstract:Objective To evaluate the feasibility on screening of diabetic retinopathy by Optomap Panoramic 200. Methods The diabetes center diagnosed with diabetes were retrospectively analyzed and participated in screening patients with clinical data(75 patients, 114 eyes). All patients were received Optomap Panoramic 200 and mydriatic ETDRS 7 -fields photography. In comparison with the sensitivity and specificity on screening of diabetic retinopathy by Optomap Panoramic 200 and the agreement between Optomap Panoramic 200 and mydrlatic 7 - fields photography. With macular hemorrhage and exudative lesions as index to evaluate the sensitivity and specificity of the diabetic macular degeneration by Optomap Panoramic 200 screening. Results With mild non - proliferative diabetic rctinopathy as the screening threshold, the sensitivity and specificity of Optomap Panoramic 200 were 73.45% and 75.01% , the consistency was 0. 625 ; With moderate non - proliferative diabetic retinopathy as the screening threshold, the sensitivity and specificity of Optomap Panoramic 200 were 94.85% and 98.82% , the consistency was 0.912. The sensitivity and specificity of macular hemorrhage by Optomap Panoramic 200 were 78.2% and 87.5% ; maeular exudative lesions were 75.4% and 86.2%. Conclusion The Optomap Panoramic 200 can become an outpatient non - mydriatie fundus photography method of screening for diabetic retinopathy.
