腹腔镜下微波消融治疗非手术适应证大肝癌疗效分析

目的:评价腹腔镜引导下微波消融作为治疗非手术适应证大肝癌的临床疗效。方法:2000年至2007年11月,我们为32例直径大于5 cm的大肝癌患者行腹腔镜引导下微波消融,肿瘤平均6.2 cm,每个肿瘤结节插入3~5针,每个病灶治疗1~3次。观察组织消融范围、肿瘤完全消融率和局部复发率。结果:肿瘤完全消融率为81.3%,局部复发率为37.5%,1年、3年、5年生存率分别为75%、59.4%、37.5%。结论:腹腔镜引导下微波消融治疗非手术适应证大肝癌,安全有效,扩大了组织的消融范围,提高了局部治疗效果。

The effect of microwave ablation in contraindicated big liver cancer through laparoscopy

Objective:To investigate the effect of microwave ablation in big liver cancer guided by laparoscopy.Methods:Thirty-two patients with big liver cancer(>5cm) underwent laparoscopic guided microwave ablation.The average diameter of tumor was 6.2cm.3-5 electrode insertions was applied for each nodule and 1-3 treatment sessions was applied for each lesion.The ablated area,complete ablation rate and local recurrence were evaluated.Results:The complete ablation rate was 81.3%,and the local recurrence rate was 37.5%.One,two and five-year survival rates were 75%,59.4%,37.5% respectively.Conclusions:The technique is safe and effective,which enlarges ablated area and improves local therapeutic effect.
