丙泊酚复合利多卡因对开颅手术应激反应的影响

目的 观察开颅手术全麻后苏醒拔管期小剂量丙泊酚复合利多卡因对应激反应的影响。方法 选择开颅手术患者30例,ASAⅠ～Ⅱ级,随机分成丙泊酚复合利多卡因组（A组）和生理盐水组（B组）,两组患者均接受芬太尼、丙泊酚、维库溴铵和吸入1.0%～1.5%异氟醚的全身麻醉,术毕A组患者静脉注射0.5mg/kg丙泊酚＋1mg/kg利多卡因,B组患者静注等容量生理盐水作对照,观察两组患者拔管前后收缩压、舒张压、心率的变化。结果 A组患者在拔管时血压、心率无明显的变化,B组患者在拔管时血压、心率明显的升高（P〈0.05）。结论 小剂量丙泊酚复合利多卡因可抑制开颅手术全麻后苏醒拔管期应激反应,平稳苏醒。

Propofol combined with lidocaine effect on the stress reaction of intracranial operation

Objective To explore the effect of low dose propofol and lidocaine on the haemodynamic response to emergence from anesthesia and tracheal extubation of intracranial operation.Methods Thirty patients were randomly allocated to group B(n=15)and group A(n=15),all patients received a standard general anesthesia with propofol,vecuronium,fentanil,1.0%~1.5% isoflueaen.At the end of surgery,propofol and lidocaine were given to group A,saline placebo was given to group B,systolic pressure,diastolic pressure and heart rate was recorded.Results Propofol and lidocaine attenuated the increase in BP and HR at sputum aspiration and extubation.Conclusions Low dose of propofol combined with lidocaine inhibits the stress reaction of intracranial operation.
