原发性眼眶静脉曲张的CT和MRI表现

摘    要:目的探讨原发性眼眶静脉曲张(POV)的CT及MRI表现。方法临床诊断的30例原发性眼眶静脉曲张患者,9例行CT横断位和冠状位扫描,21例行MR扫描,4例行CT和MR检查。结果30例中30只患眼,左眼18例,右眼12例,均为单侧发病。病变部位眼睑3例,眼眶前部1例,眼眶后部(球后)17例,眼睑和眼眶前部5例,眼睑、眼眶前部和眼眶后部2例,眼眶前部和眼眶后部2例。形态17例为不规则块状,4例为卵圆形,9例为不规则条状。CT冠状位,病变均显示为软组织密度,有明显强化;静脉石2例;眶骨受压变薄、眶腔扩大3例。MR扫描,颈部加压后25例均显示病变,11例呈等T1长T2信号,7例呈长T1长T2信号,1例呈等T1等T2信号,6例呈长T1长T2为主的混杂信号,增强的22例中,14例呈明显均匀强化,8例呈不均匀强化。结论CT横断位和冠状位及颈部加压或俯卧位MRI在原发性眼眶静脉曲张的诊断和鉴别诊断方面有重要价值。

Abstract:Objective To investigate CT and MRI findings of primary orbital varix (POV).Methods 30 cases of primary orbital varix diagnosed by clinic were analyzed retrospectively. Axial and coronal CT scan was performed in 9 patients,MR imaging was performed in 21 patients,4 patients underwent both CT and MRI examination.Results 30 unilateral eyes involved in 30 cases including 18 left ones and 12 right ones.30 lesions were located in the eyelids (3/30), anterior orbit (1/30), posterior orbit (posterior to the globe) (17/30), both eyelids and anterior orbit (5/30), both anterior and posterior orbit (2/30) , combined eyelids , anterior and posterior orbit (2/30) . The shape of lesions was irregular mass or band (17/30) (9/30) and oval (4/30).The larger soft tissue density lesions were showed on coronal CT with marked enhancement. Phleboliths were seen in two,orbital bony compression and expansion in three. MR imaging with compression of the neck veins performed in 25 patients,the lesions were identical T1 and long T2 signal intensity in 11 , long T1 and long T2 signal intensity in 7 ,identical T1 and T2 signal intensity in 1,mixed long T1 and long T2 in 6.MR contrast-enhanced scan in 22 cases showed homogeneous marked enhancement in 14 cases , and inhomogeneous marked enhancement in 8 cases . Conclusion Coronal and axial CT and MR imaging with compression of the neck veins or in the prone position are valuable in diagnosis and differential diagnosis of primary orbital varix.
