脉冲多普勒组织成像对正常右室心肌运动特点的研究

摘    要:目的应用脉冲多普勒组织成像(PW-DTI)分析正常右室心肌舒缩运动的特点.方法通过PW-DTI记录39例健康成人右室游离壁各节段以及左室游离壁基底段的运动速度曲线.结果右室游离壁基底段收缩波运动速度最高,中间段次之,心尖段最慢.舒张波运动速度基底段与心尖段有显著性差异(P<0.05),基底段与中间段、心尖段与中间段差异不显著.右室游离壁基底段收缩波峰值速度高于左室(P<0.01),收缩波加速度低于左室(P<0.01),晚期舒张波峰值速度较左室为高(P<0.01),早期与晚期舒张波峰值速度比值较左室为低(P<0.05).结论 PW-DTI技术可以准确测定右室长轴运动的速度及时间指标,是一种无创性评价右室功能的新方法.

Abstract:Objective To investigate the characteristics of right ventricular wall motion by the pulsed wave Doppler tissue imaging (PW-DTI). Methods The PW-DTI velocity curve in three segments of right ventricular (RV) free wall and the basal segment of left ventricular (LV) free wall was recorded respectively in 39 normal subjects. The peak systolic,early and late diastolic PW-DTI velocities (Vs,Ve,Va) were measured and the Ve/Va ratio was calculated. The isovolumic contraction and relaxation time intervals (ICT, IRT),the systolic and early diastolic acceleration (ACs,ACe) were also measured in the basal segment of RV and LV free wall. Results Regularity of the longitudinal motive curve of RV myocardial was found for interpretation. Vs of the basal segment was the highest among the three segments of RV free wall. Ve and Va were significantly different between the RV basal segment and the apical segment ( P =0.006,P =0.023) but no significant difference within the other segments. Some different characteristics of wall motion existed between RV and LV. Vs and Va of RV free wall were significantly higher( P =0.000,P =0.007)and ACs and Ve/Va were significantly lower( P =0.004,P = 0.024 ) than those of LV free wall. Conclusion PW-DTI can provide a quantitative,direct and accurate method for assessing RV longitudinal function.
