摘    要:Wernicke脑病(WE)是一种急性或亚急性起病的维生素B1缺乏症。表现为眼部体征、躯干性共济失调、意识障碍和情感淡漠及多发性周围神经病。M砌的表现有助于提示本病。WE是由于胃肠道手术、酗酒和胰腺炎等病因引起,补充维生素B1有助于恢复。

Abstract:Wernicke' s encephalopathy(WE) is a serious potentially fatal acute or subacute neurological disorder caused by thiamine(vitamin B1) deficiency. Thiamine, a cofactor of several metabolic enzymes, are only sufficient for up to 18 days in body for maintaining life. WE is characterized by nystagmus, abducens and conjugate gaze palsies, unsteadiness of stance and gait, confusion, and apathy. In many cases, however, the patients' clinical presentation is incomplete and there are only consciousness changes. Since pathognomonic lesions have been found in up to 2.2 % of consecutive series of autopsies in adults, it is suggested that the true prevalence of WE is clearly higher than the frequency of clinical diagnosis. Although it is most frequently observed in the patients who are chronic alcoholics, WE may also be associated with other conditions, such as protracted therapy, gastrointestinal disorders, hyperemesis gravidarum, and in any clinical setting in which thiamine is not ingested or absorbed in adequate amounts. In cases that do not correspond to the classical clinical picture, magnetic resonance imaging (MRI) may be of critical importance in the diagnosis and timely treatment of WE.
