三种不同方法在肱骨干骨折治疗中的应用及比较

为探讨3种不同固定方式在治疗肱骨干骨折中的疗效,将247例病人按来诊顺序分成3组,分别采用杉树皮夹板外固定治疗(82例)、单臂外固定支架治疗(83例)、钢板螺丝钉内固定治疗(82例),平均随访10个月.从愈合率、愈合时间、肩肘关节功能恢复情况进行分析比较,结果显示杉树皮外固定组治愈时间最短,单臂外固定支架组治愈率最高,杉树皮外固定组治愈率也明显高于钢板螺丝钉内固定组.而3种不同治疗方法对肩肘关节功能的恢复均无明显影响.认为肱骨干骨折除非有明显的手术指征,否则采用杉树皮夹板外固定治疗,不失为一种简便、安全、有效的治疗方法;若必须手术治疗首先考虑采用单臂外固定支架.

THE APPLICATION AND COMPARISON OF THREE DIFFERENCT TREATMENT METHODS IN THE TREATMENT OF HUMERAL SHAFT FRACTURE

The objective of the paper is to explore the therapeutic effect of three different fixing modes in the treatment of humeral shaft fracture. Two hundred and forty-seven cases of humeral shaft fracture were divided in order of visit into three groups: Group I (82 cases), Group II (83 cases) and Group III (82 cases), which were treated by fir bark splints external fixation, by single-arm external fixation brace and by plate screws internal fixation, respectively and followed for 10 months in average; the curative rate and healing time and the function recovery of the shoulder and elbow were analyzed and compared among the three groups. The results showed that the curative time of Group I was the shortest; the curative rate of Group II was the highest and the curative rate of Group I was significantly higher than that of Group III, too; the three different treatment methods had no significant influence on the function recovery of the shoulder and elbow. It was believed that except the case having a significant surgical indication, humeral shaft fracture should be treated by fir bark splints external fixation, which is a simple, safe and effective therapy; the single-arm external fixation brace should be first selected if the case must be treated by surgery.
