| 摘 要： | 目的 探讨免疫球蛋白重链(IgH)基因重排的检测在B细胞性淋巴瘤(B-NHL)中的诊断价值。方法 用聚合酶链式反应(PCR)方法检测B细胞淋巴瘤30例．淋巴结反应性增生(RH)10例及T细胞淋巴瘤(T-NHL)2例的IgH基因重排。结果 30例B-NHL中，22例(73．3％)出现IgH基因克隆性重排，而RH及T-NHL均未出现IgH基因重排。结论 B细胞淋巴瘤中存在B细胞单克隆增生，支持肿瘤单克隆起源学说；IgH基因克隆性重排检测对鉴别B细胞淋巴瘤和反应性增生以及T细胞和B细胞淋巴瘤有重要意义。

| Abstract: | Objective To investigate the diagnostic significance of the rearrangement of IgH gene in B cell lymphoma.Methods Specimens from 30 cases of B cell lymphoma and 10 cases of reactive hyperplasia(RH) were investigated for IgH gene rearrangements by polymerase chain reaction(PCR)technique.Results the positive amplification of clonal IgH gene in 22 out of 30 samples(73.3%)from B cell lymphoma,Monoclonality of B-lymphocytes wasn't detected in 10 cases of RH and 2 cases of T cell lymphoma.Conclusion The results provide an evidence to support the hypothesis of monoclonal origin of neoplasm. The detection of the clonal rearrangement of IgH gene may be served as a genetic marker to differentiate B cell lymphoma from RH as well as B cell lymphoma from T cell lymphoma. |
