经胸超声心动图在嵴内型室间隔缺损介入封堵中的应用

目的评价经胸超声心动图指导嵴内型室间隔缺损（IVSD）介入封堵治疗的可行性及近、远期疗效。方法对IVSD患者应用经胸超声心动图进行术前筛查,入选适宜介入封堵患者23例,术中监测封堵伞形态及瓣膜等情况,术后进行随访评价近、远期疗效。结果21例IVSD患者封堵成功,2例因放置封堵器后影响主动脉瓣开放而放弃封堵器,封堵成功率91.3%。2例出现主动脉瓣微量及少量反流,随访12个月较术后无增加。术后48 h及1、6、12个月超声心动图见封堵器形态、位置良好,未见移位及残余分流。术后左室舒张末期内径（LVEDD）较术前减少（P〈0.05）,而左室射血分数（LVEF）无显著改变（P〉0.05）。结论超声心动图在IVSD介入封堵术术前、术中及术后随访的评价中具有重要价值。

Application transthoracic echocardiography in guiding closuring of intercristal ventricular septal defect

Objective To evaluate the feasibility and therapeutic effect of transthoracic echocardiography on guiding the closuring of intercristal ventricular septal defect（IVSD）.Methods Twenty-three IVSD patients were screened carefully by transthoracic echocardiography before closuring procedure.Morphology of closure and valves were monitored by echocardiography during procedure.The patients were followed up at recently and late period.Results There were 21 cases underwent closuring procedure successfully,2 cases were quitted because theclosure of aortic valves was limited,and the success rate was 91.3%.Trivial to minimal regurgitation were found in 2 cases, and not obviously increased during 12 months follow up by echocardiography.In other cases,the morphology and position of closure was well at time point 48h,1m,6m and 12m follow up.The left ventricular end diastolic diameter （LVEDD） decreased post procedure than pre procedure（P〈0.05）.and no obviously change in left ventricular ejection fraction（LVEF）.Conclusion Echocardiography played important role in pre,intra and follow up of IVSD closuring procedure.
