Molteno引流阀植入治疗难治性青光眼

目的评价青光眼Molteno引流阀Ⅰ期植入治疗难治性青光眼的疗效。方法采用Molteno引流阀,植入颞上方球结膜下,追踪观察患者术后的视力、眼压、并发症。结果13眼中术后1月成功率为62%。10眼随访半年以上,成功率为60%。术前平均眼压(42.32±8.12)mmHg,术后1月平均眼压(14.20±8.43)mmHg,术后半年以上平均眼压(21.25±10.12)mmHg。13眼中,发现低眼压、浅前房4眼,暂时性前房出血2眼,引流管与内皮接触1眼。结论青光眼Molteno引流阀植入是目前治疗难治性青光眼的一种有效方法,术后应注意严密观察有关并发症。

Clinical observation of Molteno drainage valve implantation for the treatment of refractory glaucoma

Objective To evaluate the effect ofⅠphase Molterno drainage valve implantation on treating refractory glaucoma.Methods I phase Molteno drainage valve implantation was used to treat refractory glaucoma in13cases(13eyes).All the eyes were followed up about one month and10of the13eyes were followed up more than six months.Visual acuity,IOPs and complications were analysed.Results The curative rate of13eyes reached62%in one month postoperation,the curative rate of the ten eyes reached60%more than six months postoperation.The average IOPs of preoperation,in one month and more than six months postoperation were(42.32±8.12)mmHg,(14.20±8.43)mmHg,and(21.25±10.12)mmHg respectively.The average IOPs had significant difference between the preoperation and postoperation(P<0.01).There were4eyes of low IOPs,and flat anterior chamber,2eyes transient hyphemas and1eye of drainage tube touched corneal endothelium.Conclusion Molteno drainage valve implantation is an effective method to treating the refractory glaucoma,but its complications should be surveyed closely in postoperation.
