螺旋式导针法准确判断胸椎椎弓根螺钉植入

目的:建立判定胸椎椎弓根螺钉植入孔道是否准确的新方法.方法:在9个干燥人胸椎标本的椎弓根做通过椎弓根中央破坏椎弓根壁的孔道,然后用螺旋式导针插入孔道,由3名高年资和3名低年资骨科医生结合椎体正、侧位X光片判定定位针是侵犯椎弓根还是位于椎弓根内.结果:螺旋式导针法对于判定椎弓根螺钉孔道位置正确与否有很高的敏感性、特异性和阳性预测值.结论:螺旋式导针法是判断椎弓根螺钉植入孔道位置是否准确的有效方法.

Spiral guid pin: A new method for radiographic verification of thoracic pedicle screw pilot hole placement

Objective: To establish a new method for radiographic verification of thoracic pedicle screw pilot hole placement. Methods: The pedicle screw pilot holes were drilled within the center of the pedicle, violated pedicle walls in 9 human dried thoracic vertebrae. Kirshner wires or the spiral wires were separately placed into the holes, then the posteroanterior and lateral radiographs were taken. The radiographs were evaluated by 3 experienced orthopedists and 3 young orthopedists. After radiographs were shown to these observers, they combined the posteroanterior and lateral radiographs in each place and indicated whether the pedicle cortex was violated by the pedicle screw pilot hole or not. Results:Using spiral wires to detect pedicle pilot hole placement was higher in sensitivity,specificity and positive predictive value than other methods were. Conclusion: The technigue, spiral guid pin, detecting pedicle pilot hole placement, was effective.
