尿毒症患者心脏结构与功能改变及其临床意义

目的　初步探讨用肌钙蛋白T、超声心动图、核素心肌显像评价尿毒症患者的心肌损害及尿毒症病情的临床意义。方法　 5 3例尿毒症患者同时行肌钙蛋白T、超声心动图、核素心肌显像及肾功能检查 ,结果进行对比分析 ,部分患者透析后复查。结果　左心室舒张末内径、左心房内径及有无心包积液与肾功能指标有相关性 ,透析后左心室舒张末内径缩小。结论　尿毒症心肌损害以左心室左心房扩大及左心室肥厚多见 ,前两者可用来初步评价患者心肌损害及尿毒症病情轻重 ,透析对心肌损害有改善作用

Structural and Functional Change of Heart in Patients with Uremia and Its Clinical Significance

Objective To discuss the clinical significa nc e of troponin T(TnT),echocardiography and nuclide myocardium imaging in the eval uation on myocardial injury and the condition in patients with uremia.Me thod 53 patients with uremia were phlebotomized to measure renal functi on,TnT and TnI,at the same time echocardiography and nuclide myocardium imaging were examined.The results were analyzed comparatively.Some patients were checked up again after adequate dialysis.Results Left ventricular end -diastole diameter,left atrium diameter and the occurrence of pericardial effus ion were correlated with renal function.Left ventricular end-diastole diameter decreased after dialysis.Conclusions Left ventricle and atrium enlargement and left ventricle thickening are common in uremic myocardium.The fi rst two may be used to assess the state of myocardial injury and the illness con dition in patients with uremia.Dialysis could ameliorate the myocardial injury.
