糖尿病足感染病原菌分布与尿微白蛋白关系的临床研究#br#

摘    要:目的 探讨糖尿病足感染与尿微白蛋白水平的相关性。方法 随机选取2016年1月—2018年12月于我院内分泌科住院并确诊为糖尿病足患者79例,根据是否感染病原微生物分为感染组和非感染组,分析病原菌分布、抗菌药物敏感性及尿微白蛋白水平。结果 79例研究对象,23例培养阴性,余共培养分离出病原菌62株,革兰阴性菌32株(51.61%),革兰阳性菌28株(45.16%),真菌2株(3.22%),革兰阳性菌对万古霉素和利奈唑烷敏感率最高,革兰阴性菌对头孢吡肟、亚胺培南、头孢他啶、厄他培南敏感率最高。感染组尿微白蛋白水平高于非感染组,差异具有统计学意义。结论 糖尿病足感染病原菌种类较多,感染性糖尿病足患者以革兰阴性菌为主。尿微白蛋白水平升高可能与糖尿病足感染有关。 目的 探讨糖尿病足感染与尿微白蛋白水平的相关性。方法 随机选取2016年1月—2018年12月于我院内分泌科住院并确诊为糖尿病足患者79例,根据是否感染病原微生物分为感染组和非感染组,分析病原菌分布、抗菌药物敏感性及尿微白蛋白水平。结果 79例研究对象,23例培养阴性,余共培养分离出病原菌62株,革兰阴性菌32株(51.61%),革兰阳性菌28株(45.16%),真菌2株(3.22%),革兰阳性菌对万古霉素和利奈唑烷敏感率最高,革兰阴性菌对头孢吡肟、亚胺培南、头孢他啶、厄他培南敏感率最高。感染组尿微白蛋白水平高于非感染组,差异具有统计学意义。结论 糖尿病足感染病原菌种类较多,感染性糖尿病足患者以革兰阴性菌为主。尿微白蛋白水平升高可能与糖尿病足感染有关。

Abstract:Objective To explore the correlation between diabetic foot infections and urine microalbumin.Methods 79 patients who were hospitalized in ourdepartment of endocrinology and diagnosed as diabetic foot from January 2016 to December 2018 were randomly selected. According to whether they were infected with pathogenic microorganisms, patients were divided into the infected group and the non-infected group. We analyzed the distribution of pathogens and the sensitivity of antibiotics, and explored the correlation between diabetic foot infections and urinary microalbumin levels. Results Of the 79 subjects, 23 were negative, and 62 strains of pathogens were cultured. Among which, there were 32 strains of Gram-negative bacteria (51.61%), 28 strains of Gram-positive bacteria (45.16%), and 2 strains of fungi (3.22%). Gram-positive bacteria had the highest sensitivity to vancomycin and linezolid, while Gram-negative bacteria had the highest sensitivity to cefepime, imipenem, ceftazidime and ertapenem. The level of urinary microalbumin in the infected group was higher than that in the non-infected group, and the difference was statistically significant. Conclusion There were many types of pathogens in diabetic foot infections. Gram-negative bacteria were the main cause of infectious diabetic foot. High level of urinary microalbumin may be related with infections in diabetic foot.
