2019年北京市东城区医疗机构放射工作人员管理现状调查

摘    要:目的 了解医疗机构放射工作人员管理现状和水平,为加强监管提供依据。方法 统一设计调查表,把放射工作人员基本信息、培训、体检、持证、建档和个人剂量作为调查内容,统计方法使用χ2检验和方差分析,检验水准α = 0.05,P < 0.05为差异有统计学意义。结果 医院共有放射工作人员1209人,其中男性682人(56.4%),初级职称603人(49.9%),中级职称449人(37.1%),高级职称157人(13.0%),本科及以上学历826人(68.4%),不同医院放射工作人员的性别、职称和学历分布均不同,差异具有统计学意义(P < 0.001);趋势性χ2检验结果显示,医院级别越高,放射工作人员职称和学历越高(P < 0.001);放射工作人员培训率、个人剂量监测建档率和职业健康检查建档率均为100%,放射工作人员体检率和持证率分别为98.5%和99.6%,不同医院放射工作人员的体检率和持证率差异没有统计学意义(P > 0.05);个人剂量监测率和频次率为100.0%和93.7%;集体年剂量和平均年剂量分别为201.55 mSv·人和0.17 mSv;不同医院的平均年剂量(P < 0.001)差异有统计学意义,三级医院放射学工作人员人均年剂量显著高于其他医院(P < 0.05),不同岗位的平均年剂量(P < 0.001)差异有统计学意义,放射治疗和核医学放射学工作人员人均年剂量显著高于其他岗位(P < 0.05)。结论 医疗机构应完善放射工作人员管理,加强放射工作人员个人防护和个人剂量监测工作。

Abstract:Objective To understand the management status and level of radiation workers in medical institutions, and to provide a basis for strengthening supervision.Methods To design the questionnaire in a unified manner including the following contents: basic information, training, physical examination, certification, file and personal dose of radiation workers, statistical methods using χ2 test and variance analysis, significant level α = 0.05, P < 0.05 was statistically significant.Results There were 1209 radiation workers in the hospital, including 682 males (56.4%), 603 people (49.9%) with junior professional titles, 449 people (37.1%) with intermediate professional titles, 157 people (13.0%) with senior professional titles, and 826 people (68.4%) with bachelor degree or above. The distribution of gender, professional title and educational background of radiological staff in different hospitals was different and the differences are statistically significant (P < 0.001); the trend χ2 test results show that the higher the hospital level, the higher the titles and degree of the radiation workers (P < 0.001); The radiation staff training rate, personal dose monitoring filing rate and occupational health examination filing rate were all 100%. The radiation staff physical examination rate and certificate holding rate are 98.5% and 99.6%, respectively. There was no statistically significant difference between the physical examination rate and the certification rate of the staff in different hospitals (P > 0.05); The personal dose monitoring rate and frequency rate were 100.0% and 93.7%, respectively. The collective annual dose and the average annual dose were 201.55 mSv and 0.17 mSv, respectively. The difference in the average annual dose (P < 0.001) of different hospitals was statistically significant. The average annual dose of radiology staff in tertiary hospitals was significantly higher than that in other hospitals (P < 0.05). The difference in the average annual dose (P < 0.001) of different positions was statistically significant. The average annual dose of radiotherapy and nuclear medicine radiology workers was significantly higher than that of other positions (P < 0.05).Conclusion Medical institutions should improve the management of radiation workers and strengthen the monitoring of personal doses and personal protection.
