醒脑开窍针法联合神经肌肉电刺激治疗脑卒中后吞咽障碍的疗效观察

目的 探讨醒脑开窍针法联合神经肌肉电刺激治疗脑卒中后吞咽困难的临床疗效。 方法 选取2017年1月—2019年10月期间阜阳市人民医院及其分院收治的卒中后吞咽障碍的64例住院患者作为研究对象,随机分为对照组和实验组,对照组患者实施神经肌肉电刺激治疗,实验组患者在对照组基础上实施醒脑开窍针法治疗,2组均治疗2个疗程,共8周,治疗结束后比较2组患者的治疗有效率,观察2组患者治疗前后标准吞咽功能评估量表(standardized swallowing assessment,SSA)和美国国立卫生研究院卒中量表(NIH stroke scale,NIHSS)变化。 结果 2组有效率实验组为93.75%,对照组为71.88%,2组有效率差异有统计学意义(P<0.05);2组患者治疗前SSA量表和NIHSS量表评分差异无统计学意义(均P>0.05);治疗后2组患者SSA量表和NIHSS量表评分均较治疗前明显降低,治疗前后差异有统计学意义(均P<0.05);治疗后实验组患者SSA量表和NIHSS量表评分较对照组明显降低,差异有统计学意义(均P<0.05)。 结论 醒脑开窍针法配合神经肌肉电刺激治疗脑卒中后吞咽困难疗效显著,可明显改善吞咽障碍患者的临床症状。 

Therapeutic effect observation of Xingnao Kaiqiao acupuncture combined with neuromuscular electrical stimulation on dysphagia after stroke

Objective To explore the effect of Xingnao Kaiqiao acupuncture combined with neuromuscular electrical stimulation on dysphagia after stroke. Methods Total 64 patients with post-stroke dysphagia admitted to Fuyang People's Hospital from January 2017 to October 2019 were randomly divided into control group and experimental group. Patients in the control group were treated with neuromuscular electrical stimulation. On the basis of the control group, the patients in the experimental group were treated with Xingnao Kaiqiao acupuncture. Both groups were treated for two courses for a total of 8 weeks. After the treatment, the treatment efficiency of the two groups was compared. The changes in the standard swallowing assessment scale(SSA) before and after treatment and the National Institutes of Health Stroke Neurological Deficit Rating Scale(NIHSS) were observed. Results The effective rate of the experimental group was 93.75%, significantly higher than that of the control group 71.88%(P<0.05). There was no significant difference in the pre-treatment scores of SSA scale and NIHSS scale between the two groups(all P>0.05). After treatment, the SSA scale and NIHSS scale scores of both groups were lower than those before treatment, the difference was statistically significant(all P<0.05).The scores of the experimental group SSA scale and NIHSS scale were lower than the control group at 8 weeks after treatment, the difference was statistically significant(all P<0.05). Conclusion The effect of Xingnao Kaiqiao acupuncture combined with neuromuscular electrical stimulation on dysphagia after stroke is significant, and the clinical symptoms of patients with dysphagia are improved.
