糖尿病患者尿液转铁蛋白检测的临床意义

摘    要:为了研究尿液微量转铁蛋白(TRF)与糖尿病早期肾功能损害的关系,用速率免疫散射比浊法测定了61例非胰岛素依赖型糖尿病(NIDDM)和40例正常对照的尿液TRF。结果显示,NIDDM病人尿液TRF显著高于正常对照组(P<0.001),其增高程度与NIDDM病人年龄、病程、血糖水平、高血压和视网膜病变程度关系密切。尿液TRF与微量白蛋白(mA)和N-乙酰-β-D-氨基葡萄糖苷酶(NAG)呈显著正相关,且尿TRF升高幅度较尿mA和NAG增幅大。提示尿液TRF测定是预测NIDDM早期肾病的敏感指标。

Abstract:We studied the relationship between the urinary microtransferrin (TRF) and early glomerular damage in diabetes mellitus. 61 patients with non-insulin-dependent diabetes mellitus and 40 healthy subjects were measured for urinary TRF with rate immunonephelometry assay. The Urinary TRF concentrations were found to be greatly elevated in patients with diabetes compared with those in the health subjects (P < 0.001). The increase of TRF was closely related to age and duration of diabetes mellitus, blood glucose, hypertension, retinopathy, and it was initial parameter to predict diabetic nephropathy. There was a significant correlation among urinary TRF concentration, microalbumin, and N-acetyl-beta-D-glucosaminidase. The urinary excretion of TRF was more elevated than that of microalbumin. It is suggested that excretion of TRF in urine is a more sensitive index of the glomerular dysfunction in diabetes mellitus.
