精准肝切除技术在右半肝肝内胆管结石治疗中的应用

目的探讨采用精准肝切除技术治疗右半肝肝内胆管结石的临床效果及安全性。方法选择右半肝肝内胆管结石患者42例,按核磁共振成像系统(MRI)重建胆道、肝脏血管及肝脏三维图像,行精准肝脏切除。术后规律随访,评价术中情况、术后恢复情况及远期效果。结果 42例患者均手术成功,术后无结石残留病例,患者术后均顺利康复,术中及围术期无严重并发症发生,无死亡病例。术后规律随访3年,结石复发5例(11.9%)。结论采用精准肝切除技术治疗肝内胆管结石,手术安全,能减少结石残留、降低复发率,具有较好的临床效果。

Application of precise liver cuting on intrahepatic bile duct stone of right lobe liver

Objective To investigate the clinical effect and safety of precise liver cuting on intrahepatic bile duct stone of right lobe liver. Methods 42 cases of patients with intrahepatic bile duct stone were selected, and precise liver cuting were taken according to three dimensional picture with of biliary tract,liver and blood vessel that were reestablished with MRI, follow-up visiting were taken in postoperations, condition in operation, postoperative recovery and long-dated effect were evalua- ted. Results All operations succeed in 42 cases of patients, all patinets did not had rudimental stone, all patinets were rehabil- itated smoothly, no severe complication and death patients, all patients were follow-up visiting in three years, there were 5 cases of patients with relapsing stone. Conclusion Precise liver cuting on intrahepatic bile duct stone of right lobe liver is safe, can decrease rudimental stone and relapse, and have good clinical effect.
