腹腔镜根治术与开腹手术治疗结肠、直肠癌单中心对照研究

目的探讨腹腔镜根治术治疗结直肠癌的临床应用价值及其安全性。方法将本院2008年5月—2012年5月收治的78例结直肠癌患者随机分为2组,每组39例。治疗组采用腹腔镜根治术,对照组采用传统开腹手术,分析比较2组患者治疗前后的临床及手术资料。结果与对照组相比,治疗组的手术时间、术中出血量、术后平均住院时间、下床活动时间、术后肠道功能恢复时间、术后并发症发生率均显著降低。治疗组术后48 h、72 h血浆胃动素水平均显著高于对照组。结论腹腔镜根治术治疗结直肠癌具有出血少、恢复快、手术安全、创伤小、胃肠功能恢复快等优点,值得临床进一步推广应用。

Single center case-control study on the laparoscopic radical resection and open operation on colorectal cancer

Objective To study the clinical application value and safety of laparoscopic radical surgery on colorectal cancer.Methods Seventy-eight patients with colorectal cancer from 2008 May to 2012 May in our hospital were randomly divided into two groups.The treatment group(n=39) received the treatment of laparoscopic radical resection,and the control group(n=39) received traditional open operation.The clinical and operation data of the two groups were analyzed and compared.Results Compared with the control group,the indices of operation time,bleeding volume in operation,the mean postoperative hospital stay,ambulation,postoperative intestinal function recovery time,postoperative complication rate in the treatment group were significantly lower.The levels of plasma motilin 48 h and 72 h after operation in the treatment group were obviously higher than that in the control group.Conclusion Laparoscopic radical surgery on colorectal cancer has the advantages of less bleeding,quick recovery,safe operation,less trauma,quick recovery of gastrointestinal function and it is worthy of further clinical application.
