基层医院开展原发性肝癌外科治疗105例分析

目的探讨基层医院开展肝癌外科治疗的临床经验。方法回顾性分析本院1995—2012年收治的105例原发性肝癌患者的手术治疗情况及随访。其中选择性收住的原发性肝癌101例,急诊肝癌破裂大出血4例;行左半肝切除32例,肝部分切除53例,右半肝切除17例,单纯肝动脉结扎3例。结果手术切除率97.1%(102/105),死亡率1.9%(2/105);临床治愈率59.0%(62/105)。结论基层医院可根据自身条件开展肝癌外科治疗,尤其在病例选择、专业技术、手术各环节等方面做好准备,可满足相当一部分低收入肝癌患者的治疗需求。

Clinical analysis of surgery in the treatment of 105 patients with primary liver cancer in primary level hospitals

Objective To explore the clinical experience of surgical treatment for primary liver cancer in primary level hospitals.Methods A total of 105 patients with primary liver cancer who were admitted to our hospital from 1995 to 2012 were included in the study.The surgical treatment and follow-up data were retrospectively analyzed.Among them,101 patients with primary liver cancer were selectively admitted,and 4 patients received emergency treatment because of rupture and bleeding.Thirty-two patients underwent left hemihepatectomy,53 underwent partial hepatectomy,17 underwent right hemihepatectomy,and 3 underwent hepatic artery ligation.Results The results were as follows.The rate of surgical resection was 97.1%(102/105),mortality rate was 1.9%(2/105) and the clinical cure rate was 59.0%(62/105).Conclusion Primary level hospitals can selectively carry out surgical treatment according to their own conditions,especially in the case selections.The good technological preparation and overall preparation can meet the demands of a substantial amount of people with low-incomes.
