氟比洛酚酯联合利多卡因预防拔管期应激反应的临床观察

目的观察氟比洛酚酯联合利多卡因预防全麻拔管时应激和镇痛效果。方法 60例ASAⅠ～Ⅱ级择期手术患者,随机分为2组,对照组(Ⅰ)和观察组(Ⅱ、Ⅲ、Ⅳ),在手术开始缝皮时停麻醉药,各组给予利多卡因(1 mg/kg)和/或氟比洛酚酯(2 mg/kg),后具备拔管指针时拔管,记录麻醉前、拔管前、拔管后1、5和10 min的血压和心率,并计算收缩压和心率乘积(RPP),同时观察术后并发症情况。结果麻醉前和拔管前的心率血压无显著差异,在拔管后1和5 minⅡ、Ⅳ组的血流动力学指标更平稳(P<0.05),拔管后10 minⅢ、Ⅳ组患者较对照组疼痛明显减轻(P<0.05),而各组术后并发症情况无差异。结论氟比洛酚酯联合利多卡因应用于全麻拔管的心血管应激反应的预防有效且安全。

Effect of flurbiprofen axetil with lidocaine on preventing ardiovascular stress response in extubation period

Objective To investigate the effect of flurbiprofen axetil with lidocaine on preventing cardiovascular stress response in extubation period.Method Sixty ASA class Ⅰ or Ⅱpatients undergoing elective cholecystectomy under anesthesia were randomly assigned into two groups of Ⅰ,Ⅱ, Ⅲ and Ⅳ with 15 cases each.When the surgeon began to suture the incision,flurbiprofen axetil 2 mg/kg and(or) lidocaine 1 mg/kg were administered.Heart rate,blood pressure,RPP in several time points were recorded and pain scores,extubation time,recovery time and side effects were observed.Result The emodynamics was more stable in groups Ⅱ and Ⅳ than group Ⅰ during the extubation period(P<0.05).Pain scores were significantly lower in groups Ⅲ and Ⅳ compared to group Ⅰ(P<0.05).There was no significant difference in the incidence of nausea and vomiting between the four groups.Conclusions Flurbiprofen axetil with lidocaine can effectively prevent the cardiovascular stress response during extubation period,and analgesia after general anesthesia.
