辛伐他汀对高血压合并颈动脉硬化患者肱踝脉搏波传导速度及颈动脉内膜中层厚度的影响

摘    要:目的探讨辛伐他汀对高血压合并颈动脉硬化患者肱踝脉搏波传导速度(baPWV)及颈动脉内膜中层厚度(IMT)的影响。方法 120例原发性高血压合并颈动脉硬化患者,随机分为辛伐他汀组和对照组,各60例。2组患者均维持原有降压方案不变,治疗组在原有降压方案基础上给予辛伐他汀治疗。观察2组患者治疗前后的血脂、IMT及baPWV变化情况。结果与治疗前比较,辛伐他汀组治疗后血清TC、TG、LDL-C水平明显降低(P<0.01),血清HDL水平明显升高(P<0.01);治疗后,辛伐他汀组血清TC、TG、LDL-C水平明显低于对照组(P<0.01),血清HDL水平明显高于对照组(P<0.01)。与治疗前比较,2组治疗后baPWV明显降低(P<0.01),辛伐他汀组治疗后IMT明显减少(P<0.01);与对照组治疗后比较,辛伐他汀组IMT、baPWV水平明显增加(P<0.01)。结论辛伐他汀可有效降低血脂水平,通过降低IMT及baPWV防止动脉粥样硬化形成。

Abstract:Objective To explore the impact of simvastatin on brachial-ankle pulse wave(baPWV) velocity and carotid intima-media thickness(IMT)in hypertension patients combined with carotid atherosclerosis.Methods A total of 120 hypertension patients complicated by atherosclerosis were divided into simvastatin group(n=60) and control group(n=60).Both groups sustained original plan on hypertension,and the treatment group received additional simvastatin therapy on original plan.The changing of blood lipid,IMT,baPWV pretherapy and posttheraphy were observed.Results Compared with pretherapy,TC,TG and LDL-C level were significantly decreased in simvastatin group(P<0.01),and HDL level was significantly increased.After treatment,TC,TG and LDL-C level were significantly lower in simvastatin group than in the control group(P<0.01).HDL level in simvastatin group was higher than in the control group(P<0.01).Compared with pretherapy,baPWV level decreased in both groups,and IMT level significantly lessened in simvastatin group(P<0.01).Compared with posttherapy control group,IMT and bPWV level of simvastatin increased significantly(P<0.01).Conclusion Lessening IMT level and baPWV to prevent atherosclerosis and simvastatin can significantly decrease the blood lipid level.
