彩色多普勒超声检查在甲状腺肿块诊断中的价值

摘    要:目的回顾性分析389例经手术后病理证实的甲状腺肿块的超声声像图特点,探讨彩色多普勒超声对甲状腺肿块诊断的价值。方法应用二维超声及结合彩色多普勒超声对389例甲状腺肿块患者进行检查并作出超声诊断,将其结果与术后病理检查结果对照。超声诊断都以肿瘤的形态、边界、包膜、内部回声、后方回声、硬度和颈部淋巴结有无转移及彩色多普勒超声显示甲状腺肿块的血流情况等指标作综合分析。结果本组389例甲状腺肿块手术后经病理证实为甲状腺癌47例,超声诊断甲状腺癌39例,符合率为82.9%;甲状腺瘤172例,超声诊断甲状腺瘤159例,符合率为92.4%;结节性甲状腺肿170例,超声诊断为161例,符合率为94.7%。结论超声对甲状腺肿块的诊断有一定的规律性和特异性,结合彩色多普勒超声血流显像(CDFI)能提高其诊断符合率,减少漏诊、误诊。

Abstract:Objective To debate the value of color doppler uhrosonography in the diagnosis of goiter based on the retrospective analysis of the ultrasound sonogram feature of 389 cases with goiter testified by post-operative pathology. Methods Two-dimensional ultrasound and color Doppler ultrasound were carried out on the 389 cases with goiter to achieve ultrasound diagnosis, then these data were compared with the result of post-operative pathology. All the indexes aquired from ultrasound were generally analyzed, such as tumor shape, border, coated, internal echo, rear echo, hardness and neck lymph node with metastasis or not, blood flow condition by color Doppler ultrasound, and so on. Results Among 389 cases with goiter, 47 cases were diagnosed with thyroid carcinoma by post-operative pathology. 39 cases were diagnosed with thyroid cancer by ultrasound, the coincidence is 82.9%. 172 cases with thyroid adenoma by pathology and 159 cases by ultrasound, the coincidence is about 92.4%. 170 case with nodular goiter and 161 were diagnosed by ultrasound ,the coincidence is about 94.7%. Conclusion Ultrasound has somewhat specificity and regularity in diagnosing goiter, which can achieve the higher coincidence, reduced rate of missed diagnosis and misdiagnosis by CDFI.
