双侧平衡去骨瓣减压治疗重型闭合性颅脑损伤的疗效观察

目的探究双侧平衡去骨瓣减压治疗重型闭合性颅脑损伤的效果。方法2007年8月-2012年8月本科收治的重型闭合性颅脑损伤患者220例，分为两组，A组120例行一次双侧预见性去大骨瓣减压术，B组100例分次行双侧去骨瓣减压术，分析并比较两组患者的预后情况。结果A组死亡24例（20．0％），B组死亡32例（32．0％），两组差异有统计学意义（P〈0．05）；A组术后恢复良好率（36例，30．0％）高于B组（17例，17．0％）（P〈0．05）。结论预见性一次双侧去大骨瓣减压治疗重型颅脑损伤疗效满意，值得临床推广。

The curative effect observation of serve closed heads injuries with bilater- al decompressive craniectomy

Objective To explore the curative effect of the bilateral decompressive craniectomy on serve closed heads injuries. Methods 220 patients with serve closed heads injuries were selected in our hospital from Aug 2007 to Aug 2012. They were divided into group A and group B. 120 cases of group A were treated by active bilateral decompres- sive craniectomy completely by an operation and other 100 ones were by passive bilateral decompressive craniectomy in？bursts of group B. The prognosis were analyzed and compared between two groups. Results The mortality in group A （20.0%） was lower than that of group B （32.0%） （P 〈 0.05）. The percent of patients with good prognosis in group A （36.0%） was higher than that in group B （17.0%） （P 〈 0.05）. Conclusion The clinical effect of bilateral decompressive craniectomy on treatment of serve heads injuries is satisfactory and it is worthy of promoting.
