颅脑疾病患者采用密闭式与开放式吸痰法的效果观察

目的分析颅脑疾病患者采用密闭式与开放式吸痰法的效果,以探讨颅脑疾病患者行机械通气期间最佳的吸痰方式。方法将80例行机械通气的颅脑疾病患者随机分成两组,观察组采用密闭式吸痰,对照组采用开放式吸痰,比较两组患者吸痰前后潮气量、气道压力,并通过监护仪记录血氧饱和度、心率、血压的变化情况。结果观察组患者吸痰后,各项呼吸机监测参数及SpO2改善,患者心率、血压与吸痰前差异无统计学意义;对照组吸痰后,呼吸机监测参数及SpO2与吸痰前差异无统计学意义,而吸痰前后患者心率、血压明显改变,差异有统计学意义。结论颅脑疾病患者行机械通气期间采用密闭式吸痰,可以减少气道刺激及不良反应,是一种更安全有效的吸痰方式。

A comparative study on two sputum suction methods between open type and close type in patients with cerebral disease

Objective To observe the nursing effects of two sputum suction methods between open type and close type in patients with cerebral disease. Methods 80 cases of patients on mechanical ventilation were randomly divided into two groups,40 patients in each group. Experiment group were performed with close type of aspiration of sputum , Others were performed with open type of aspiration of sputum. To observe VT, airway pressure, SpO2 , heart rate and mean blood pressure after suctioning between two groups. Results In experiment group, the respiratory monitoring indicators and SpO2 improved significantly after suction, but the difference of heart rate and blood pressure were not significant compared with that before suction. In control group, the respiratory monitoring indicators and SpO2 were not significantly improved after suction, but the difference of heart rate and blood pressure were significant compared with that before suction. Conclusion Close type of aspiration of sputum via open type in patients with cerebral disease on mechanical ventilation can reduce airway stimulus and incidence rate of side effect ,is effective and safe.
