不同手术方法治疗粘连性肠梗阻的效果比较

目的探讨不同手术方式治疗粘连性肠梗阻的临床效果，旨在为该病临床治疗提供理想的治疗方案。方法选取本院2011年1月～2013年1月收治的85例粘连性肠梗阻患者为研究对象，根据患者治疗意愿随机将患者分为常规手术治疗组（对照组）40例以及行腹腔镜手术治疗组（观察组）45例，对比分析两种手术方法的治疗效果及术后对患者生活质量的影响。结果观察组患者治愈率及总有效率分别为71．11％、97．78％，对照组治愈率及总有效率分别为60．00％、87．50％，两组患者治愈率及总有效率差异有统计学意义（P〈0．05）。观察组患者术中出血量、平均手术时间、平均住院时间显著少于对照组，差异有统计学意义（P〈0．05）。术后1个月对两组患者进行QOL生活质量评分，观察组食欲、精神、睡眠、疼痛、疲乏、日常生活以及总评分均优于治疗前及对照组，差异有统计学意义（P〈0．05）。结论经腹腔镜手术治疗粘连性肠梗阻患者临床治疗效果理想，术中出血量较少，安全可靠，有利于提高患者术后的生活质量。

Effect comparison of different operation methods in the treatment of ad- hesive ileus

Objective To explore the clinical effects of different operation methods in the treatment of adhesive ileus in order to provide the optimum therapeutic regimen for the disease in clinic.Methods Eighty five patients with adhe- sive ileus admitted into our hospital from January 2011 to January 2013 were selected and randomly divided into regu- lar operation group （control group,n=40） and laparoscopic surgery group （observation group,n=45）.The therapeutic ef- fects of the two operation methods and conditions influencing on patient＇s life were compared in both groups.Results The cure rate and total effective rate in the observation group were 71.11% and 97.78% respectively,whereas in the control group,it was 60.00% and 87.50% in turn,which displayed statistically significant difference （P〈0.05）.The intra- operative blood loss,average operation time,and average stay in the observation group were greatly shorter than those in the control group with statistical differences （P〈0.05）.One month after surgery,the quality of life （QOL） score was ap- plied.The parameters of appetite,vitality,sleep,pain,lassitude,daily life,and total score in the observation group were su- perior to those before treatment and those in the control group,which displayed statistical differences （P〈0.05）.Con- clusion Application of laparoscopic surgery in the treatment of adhesive ileus can obtain an optimum clinical effect at advantages of little intraoperative blood loss,safety,and reliability,which is beneficial to improve patient＇s QOL after op- eration.
