降钙素原在细菌感染患儿中的临床价值

目的探讨血清降钙素原（PCT）在儿童细菌感染中的临床价值。方法回顾性分析2010年9月～2012年1月本院儿童重症监护室收治的150例脓毒症危重患儿的临床资料．其中，细菌感染组60例、病毒感染组36例、病因不明感染组54例。全部患儿于入院治疗前（R）及使用抗感染药物24h（TM）、96h（氏）分别检测血清PCT的水平。结果在抗感染治疗前，血清PCT在病因不明感染组中为（1．83±0．25）ng／ml，在病毒感染组中为（0．45±0．12）ng/ml，在细菌感染组中为（4．38±0．39）ng/ml；细菌感染组中血清PCT水平明显高于病毒感染组和病因不明感染组，差异有统计学意义（P〈0．05）；动态监测全部患者的PCT水平发现，细菌感染组在抗感染治疗后血清PCT快速下降，而病毒感染组和病因不明感染组PCT水平仍维持在原有水平，其回落变化速度明显慢于细菌感染组（P〈0．05）。结论血清PCT与细菌感染具有良好的相关性，具有对病情的早期评估、预后评价及疗效观察的价值。

The clinical value of serum procalcitonin in children with bacterial infec- tion

Objective To evaluate the clinical values of blood procalcitonin （PCT） in cases of bacterial infection in children. Methods The clinical date of 150 children with bacterial infection who were admitted to the ICU from September 2010 and January 2012 were analyzed retrospectively.All patients were determined the level of serum PCT at the time of admission （T0）,after the use of antibiotics for 24 hours （T24）, 96 hours （T96）. Results Before the application of antibiotics,serum PCT levels of etiology not established group was （1.83±0.25） ng/ml,（0.45±0.12） ng/ml for viral infec- tion group,（4.38±0.39） ng/ml for bacterial infection group.Serum PCT levels of bacterial infection group were signifi- cantly higher than that of etiology not established group and viral infection group and there existed the significant dif- ferences among the three groups （P〈0.05）.Dynamie monitor revealed that serum PCT of bacterial infection group can rapidly decrease after controlling the infection,while viral infection group and etiology not established group remain for- mer level.Their fall back rate was lower than that of bacterial infection group. Conclusion The serum PCT proves to be associated with the bacterial infection,which has an important value in early evaluation,prognosis and curative effect.
