自体荧光支气管镜检查在肺部结节或肿块诊断中的应用

目的评价自体荧光支气管镜(AFB)检查在肺部肿块或结节性病变中的诊断价值。方法将临床和放射学检查后须行支气管镜检查的38例患者应用AFB采用多种取材方法测定的诊断阳性结果与43例应用白光支气管镜(WLB)采用同样取材方法检测的诊断阳性结果进行比较。结果38例患者应用AFB钳取活检、针吸、刷检和灌洗后,共同明确诊断33例,综合诊断阳性率为86.8%,刷检、灌洗与联合诊断阳性率比较差异有统计学意义(P<0.01);应用AFB使钳取、针吸、联合诊断阳性率较应用WLB诊断阳性率明显提高,差异有统计学意义(P<0.01)。结论应用AFB通过钳取活检、刷检、针吸及灌洗多种取材联合应用的方法,使检出肺癌的综合诊断阳性率明显提高,表明AFB对肺癌的诊断具有更重要的临床应用价值。

Application of Autofluorescence Bronchoscopy in Diagnosis of Pulmonary Nodules or Masses

Objective To evaluate the clinical effect of autofluorescence bronchoscopy(AFB)in diagnosis of lung tumor or nodular lesions.Methods Positive rates of 38 cases diagnosed by AFB with multiple sampling methods and 43 cases diagnosed by WLB with the same sampling ways were compared.Results 33 cases were diagnosed in 38 patients sampled by forceps,needles,brushing and douche with AFB,comprehensive positive diagnosis rate was 86.8%.Comparing the positive rates of getting samples by brush biopsy and douche with comprehensive positive rate,the difference was of statistical significance(P<0.01).Getting samples by forceps,needles with AFB,comprehensive positive rate was much higher than with WLB and there was significantly statistical difference(P<0.01).Conclusion By AFB and combining methods of forceps biopsy,brush biopsy,needle aspiration and douche to get samples,the comprehensive positive diagnosis rate of lung cancer has been obviously improved,which has demonstrated that AFB has more significant clinical value for diagnosis of lung cancer.
