高血压病伴高尿酸血症与心力衰竭关系的研究

目的:探讨高血压病(EH)伴高尿酸血症与心力衰竭(HF)发生及程度的关系。方法:197例EH患者按血尿酸水平分为两组,高尿酸血症(HUA)组103例,血尿酸正常(NUA)组94例。对两组的HF发生率及分级、高血压分级、血中尿酸(UA)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、甘油三酯(TG)、血糖(Glu)浓度进行分析和比较。结果:HUA组的HF发生率、血中UA、TC、LDL-C、TG和Glu浓度均高于NUA组(P<0.05);EH伴HUA组的HF有症状组血UA浓度高于无症状组(P<0.01)及高血压3级(P<0.05);HUA组高血压3级的血UA浓度高于1级、2级(P<0.05);EH患者的收缩压和舒张压与血UA浓度呈正相关(P<0.05)。结论:伴有HUA的EH患者与HF的发生和程度有较密切的相关关系,因此,血UA可作为EH患者评估HF病情进展的一个有意义的生化指标。

A Study on the Relationship between Hyperuricemia and Heart Failure in Patients with Essential Hypertension

Objective:To investigate the relationship between hyperuricemia and the incidence of heart failure(HF)in patients with essential hypertension(EH).Methods:197 patients of EH were divided into two groups according to the serum uric acid level,103 patients were hyperuricemia(HUA)group,94 patients were normouricemia(NUA)group.The incidence and extent of HF,uric acid(UA),total cholesterol(TC),high-density lipoprotein cholesterol(HDL-C),low-density lipoprotein cholesterol(LDL-C),triglyceride(TG)and glucose(Glu)in serum of these two groups were analysed and compared.Results:The incidence of HF,the concentrations of UA,TC,LDL-C,HDL-C,TG and Glu in HUA group were higher than those in NUA group(P<0.05).The concentration of UA in symptomatic group of HF was higher than that in asymptomatic group of HF(P<0.01)and stage 3 of hypertension(P<0.05).The concentration of UA in stage 3 of hypertension in HUA group was higher than that of stage 1 and 2(P<0.05).There were positive correlations between concentration of UA and systolic pressure,diastolic pressure in patients with EH(P<0.05.Conclusion:The incidence and extent of HF is closely related to HUA in patients with EH,therefore,the concentration of UA may be used as a significant index for the evaluation of development of HF in the EH patients.
