1136周期宫腔内夫精人工授精临床因素的分析

目的:对影响宫腔内夫精人工授精(Intrauterine insemination,IUI)妊娠率的因素进行分析。方法:对接受夫精宫腔内人工授精治疗的651例不孕患者1 136周期的临床资料进行回顾性分析,主要分析不同的促排卵方案、IUI周期数、子宫内膜形态、内膜厚度等与妊娠率的关系。结果:1 136周期的IUI中妊娠周期93周期,周期妊娠率8.19%;<30岁、30～35岁,和>35岁妊娠率分别是8.80%、7.69%和5.42%;不同的促排卵方案(CC、hMG/hCG、hCG)的妊娠率分别为10.00%、9.00%和7.46%;hCG日内膜厚度为<8 mm、8～14 mm、>14 mm,妊娠率分别为5.61%、8.70%和12.70%;hCG日或出现LH峰当日子宫内膜形态为A型、B型、C型时,妊娠率分别是9.63%、4.55%和8.00%;重复周期IUI对提高妊娠结局差异无统计学意义。结论:施行IUI女方的年龄是影响IUI妊娠率的主要因素;多次重复周期IUI并不能显著改善妊娠结局;不同促排卵方案的妊娠结局差异无统计学意义。

Analysis on pregnancy outcome in 1136 cycles of intrauterine insemination

Objective:To analyse the effect factors of pregnancy rate after intrauterine insemination(IUI) with husband's sperm.Methods:Clinical data of 651 couples undergoing 1 136 IUI cycles were analysed retrospectively.The relationship between different ovulation induction protocols,cycles of IUI,endometrial patterns,endometrial thickness and pregnancy rate were observed.Results:93 pregnancy cycles among 1 136 cycles of IUI were found,The cycle pregnancy rate was 8.2%.The pregnancy rates of women below 30 years,aged 30~35 years old and beyond 35 years were 8.80%,7.69% and 5.42%;the pregnancy rates of different ovulation induction protocols(CC,hMG/hCG,hCG) were 10.00%,9.00% and 7.46%;the pregnancy rates of different endometrial thickness(<8 mm,8~14 mm,>14 mm) at the day of hCG injection were 5.61%,8.70% and 12.07%;the pregnancy rates of different endometrial patterns(A,B and C) at the day of hCG injection or LH surge were 9.63%,4.55% and 8%;there was no correlation between high repeat intrauterine insemination cycles and pregnant outcomes.Conclusion:The age of female patients is the major effect factor of pregnancy rate;high repeat intrauterine insemination cycles can't improve the pregnant outcomes;There is no correlation between ovulation induction protocols(CC,hMG/hCG,hCG) and pregnant outcomes.
