丙泊酚复合瑞芬太尼全身麻醉在妇科腹腔镜手术的临床观察

目的:比较微量泵输注丙泊酚复合瑞芬太尼与静吸复合两种麻醉方法用于妇科腹腔镜手术中的麻醉效果。方法:妇科腹腔镜手术患者64例,随机分为两组(n=32),瑞芬太尼组(R组):丙泊酚复合瑞芬太尼静脉麻醉,芬太尼组(F组):安氟醚复合芬太尼静吸麻醉。监测麻醉诱导、气管插管、切皮时的血压、心率的变化,记录自主呼吸恢复时间、睁眼时间、拔除气管导管时间等。结果:R组诱导期心率较F组明显下降(P<0.05);R组术后睁眼时间、呼吸恢复时间、拔管时间均较F组明显缩短(P<0.05);两组恶心、呕吐发生率无明显差别。结论:行妇科腹腔镜手术,微量泵输注丙泊酚复合瑞芬太尼麻醉比常规静吸复合麻醉更加平稳,苏醒迅速,更加有效安全。

Clinical Observation of the Propofol-Remifentanil General Anaesthesia Applied in Aynecological Laparoscopic Operation

Objective To compare the anesthetic effects by the infusion of propofol-remifentanil in minim pump and the combined intravenous inhalation anesthesia applied in gynecological laparoscopic operation.Methods 64 patients undergoing gynecological laparoscopic operation were separated into two groups randomly(n=32).Group R were given the propofol-remifentanil intravenous anaesthesia.Group F were given the enflurane- fentanyl combined intravenous inhalation anesthesia.Monitoring the blood pressure and heart rate when anesthetic induction,intubation,begining and exploration of operation.the patients' in-dependent recovery time,eye opening time upon calling and extubation time after the drug withdrawal were recored.Results HR were significantly lower in Group R than those in Group F when anesthetic induction(P<0.05).Group R were remarkably shorter at the in-dependent recovery time,eye opening time and extubation time was remarkably shorter(P<0.05).the occurrence of postoperative nausea and vomiting has no obvious difference.Conclusion Comparing with the regular intravenous inhalation anesthesia in gynecological laparoscopic operation,the anesthesia by infusing propofol-remifentanil inminim pump is more stable.It makes the patients wake up more quickly and is more effective and safer.
