先天性胆管囊肿43例诊治体会

目的 探讨先天性胆管囊肿的诊断及治疗经验。方法 对该院18年来43例先天性胆管囊肿的诊断、治疗及疗效进行回顾性分析。结果 先天性胆管囊肿具有典型的腹痛、黄疽及腹部包块三联征者少，仅12％，合并肝胆胰疾病者多，高迭74％。临床Ⅰ型较多见，B超、CT和MRCP均有重要的诊断价值。首选术式为囊肿切除，肝管空肠Roux-en-y吻合术，术后并发症少，效果良好。结论 先天性胆管囊肿一经确诊，应尽早手术治疗，首选术式为囊肿切除，肝管空肠Roux-en-y吻合术。

Diagnosis and treatment of congenital choledochal cyst

[Objective] To evaluate the diagnostic and therapeutic experience for congenital eholedoehal cyst. [Methods] The clinical data of 43 patients were analyzed retrospeetivel. [Results] The patients with congenital eholedoehal cyst presented an abdomined pain, tumour and ietenls was seldom, only 12%. It can be improved up to 74% when with liver-gallbladder-pancreas infections. Type 1 cyst was the dominant type of the cyst, uhrasonography, Computerized Tomography and MR of Choledoehal Paneraeatie Duct are of high value in diagnostic. Excision of the bile duct cyst and roux-en-y hepatieojejunostomy was the best choice of surgical management for the cyst patients, the postoperative complication is low, good effect, the result is satisfactory. [ Conclusion] Once the diagnosis is confirmed, the patients should resort to surgical treatment as soon as possible, Excision of the bile duct cyst and roux-en-y hepaticojejunostomy was the best choice of surgical management for the cyst patients.
