2型糖尿病并发下肢血管病变患者IL-6启动子单核苷酸多态性研究

目的探讨IL-6启动子-174G/C和-572C/G的单核苷酸多态性频率分布与2型糖尿病患者下肢血管病变的关系。方法 280例中国北方地区汉族成年2型糖尿病患者,根据临床诊断分为下肢血管病变(VDLE)组、下肢神经病变(NDLE)组、下肢血管-神经病变(VNDLE)组和无下肢并发症(NC)组,另选70例社区查体正常成人作为对照(CON)组。采用PCR-RFLP法测定IL-6启动子区-174G/C和-572C/G单核苷酸多态性;采用ELISA法测定血清IL-6、IL-6受体和hsCRP。结果 IL-6启动子的-174G/C基因型及等位基因在2型糖尿病组和对照组的频率分布差异均无统计学意义。而IL-6启动子-572C/G在2型糖尿病组中的基因型GG和CG、等位基因G均高于对照组(P0.05),并且VDLE组和VNDLE组高于其他组(P0.05)。结论 2型糖尿病下肢血管病变与IL-6启动子的-174G/C单核苷酸多态性可能无关,而与IL-6启动子区-572C/G单核苷酸多态性可能有关。IL-6启动子区-572C/G的GG基因型和G等位基因可能是2型糖尿病发生下肢血管病变的易患基因。

The Polymorphism of the Interleukin-6 Promoter in Patients with Type 2 Diabetes Mellitus Complicated with Low-extremity Vascular Disease

Objective To investigate the single nucleotide polymorphism of IL-6 promoter -174 G/C and -572C/G in type 2 diabetic patients and their association with lower extremity vascular disease. Methods Based upon the clinical manifestation and the ultrasound test of the lower extremities, 280 type 2 diabetic patients were divided into four groups： Vascular disease （VDLE） group, Neuropathic disease （NDLE）group, Vascular and Neuropathic disease（VNDLE） group, and no complication （NC） group.And 70 healthy people were recruited as control （CON）. By PC R-RFLP technique, the polymorphism of the human IL-6 gene-174 G/C and -572C/G was evaluated.Also, the concentration of serum IL-6, IL-6 receptor and hs-CRP were measured by ELISA technique. Results There was no statistical difference Of the IL-6 promoter-174G/C genotypes and alleles in the type 2 diabetes mellitus group and CON group （P〉0.05）. The genotype GG and CG and the allele G of IL-6 promoter-572C/G seemed to be more frequent in VDLE and VNDLE group than that in NC group as well as in the CON group （P〈0.05）. Conclusion The polymorphism of the IL-6 promoter-572C/G but not the -174 G/C might be associated with the type 2 diabetes mellitus with low-extremity vascular disease, while the genotype GG and allele G of the IL-6 promoter -572C/G might be the susceptibility genes.
