婴幼儿支气管肺炎患儿血清心肌酶变化及其临床意义

摘    要:目的研究婴幼儿支气管肺炎患儿血清心肌酶的变化,以便在诊治过程中采取综合措施。方法选择住院支气管肺炎患儿110例,其中轻症支气管肺炎80例(轻症肺炎组),重症支气管肺炎30例(重症肺炎组)。另选22例健康儿童作为对照组。采用Olympus AU2700型全自动生化分析仪测定支气管肺炎患儿急性期、恢复期及对照组儿童血清天门冬氨酸转氨酶(AST)、乳酸脱氢酶(LDH)、α-羟丁酸脱氢酶(α—HBDH)、肌酸激酶(CK)及其同功酶(CK—MB)等多项心肌酶指标。结果与对照组相比,轻症及重症肺炎组患儿血清AST、LDH、α—HBDH、CK、CK—MB水平均明显升高(P〈0.01),重症肺炎组上述指标明显高于轻症肺炎组(P〈0.01)。与急性期相比,恢复期这些心肌酶指标均趋向正常水平。结论婴幼儿支气管肺炎患儿血清心肌酶有不同程度升高,病情越重,心肌酶升高越明显。

Abstract:Objectives To investigation the variations of blood serum myocardial enzymes in infants and young children with bronchial pneumonia so as to take comprehensive measures in diagnosis and treatment. Methods 110 hospitalized infants and young children of 1 month to 3 years suffering from bronchial pneumonia were included in this study, of whom 80 cases had mild conditions (mild group ) and 30 cases had severe conditions (severe group ). 22 healthy young children were designated as control group. An Olympus AU 2700 auto - analyzer was used to determine the myocardial enzyme indexes in the acute stage and eonvaleseence, including aspartate aminotransferase (AST) , lactate dehydrogenase (LDH), α- hydroxybutyrase dehydrogenase (α- HBDH) , creatine kinase (CK) and creatine kinase myocardial bound ( CK - MB) in the young patients. Results Compared with the control group, the levels of AST, LDH, α - HBDH, CK and CK - MB in both the pneumonia groups (P 〈 0.01 ). The above indexes in the severe group were markedly higher than the mild group (P 〈 0.01 ). Compared with the acute stage, all the myocardial enzyme indexes in the convalescence tend to be normal. Conclusion The blood serum myocardial enzymes of infants and young children with bronchial pneumonia increase in different degrees. There is a positive correlation between the severity and the levels of myocardial enzymes.
