芬太尼、瑞芬太尼分别复合异丙酚麻醉用于老年无痛胃镜检查的比较研究

目的观察芬太尼、瑞芬太尼分别复合异丙酚在老年无痛胃镜检查中麻醉的效果及对呼吸、循环功能的影响,并进行比较。方法接受无痛胃镜检查的120例老年患者（ASAⅠ-Ⅱ级）随机分为两组：异丙酚复合芬太尼（Ⅰ）组,异丙酚复合瑞芬太尼（Ⅱ）组,每组各60例。记录各组检查前基础值及检查中、检查后的心率（HR）、血氧饱和度（SpO2）、收缩压（SBP）、舒张压（DBP）;观察检查中患者反应及检查后有无反应。结果两组均能达到满意麻醉效果,但Ⅱ组与Ⅰ组相比,异丙酚的总用量较少,对患者呼吸、循环功能的抑制作用较小,苏醒快,检查中、后出现不良反应的患者数较少。结论采用瑞芬太尼复合异丙酚麻醉用于老年无痛胃镜检查好于芬太尼复合异丙酚,既有良好的麻醉效果,又有很好的安全性,是较为理想的麻醉方法。

Compare clinical effect on anesthesia with remifentanil or fentanyl combined with propofol for aged people's painless gastroscopic

Objective To observe and compare the clinical effect of fentanyl and remifentanil com- bined with propofol in aged people＇s painless gastroseopie. Methods 120 cases aponia-gastroseopie with ASA Ⅰ -Ⅱ were randomly divided into fentanyl combined with propofol （ Ⅰ ）group and remifentanil combined with propofol（ Ⅱ ） group,60 cases everygroup. The data of HR, SpO2 , SBP, DBP and reaction of patients before, in and after painless gastroseopie were observed. Results Anesthesia effect of two groups are gratification, but propofol dosage,inhibitory action for patients＇ breathing and circulation palinesthesia time and untoward reaction cases of Ⅱ group is less than Ⅰ group. Conclusion Compared with fentanyl combined with propofol, remifentanil combined with propofol can be used more effectively and safely in aged people＇s painless gastroseopie. It＇s a ideal anesthesia method.
