曲安奈德染色玻璃体联合高速玻切治疗增殖性糖尿病视网膜病变

摘    要:目的 探讨曲安奈德染色玻璃体联合高速玻璃体切除(玻切)在增殖性糖尿病视网膜病变(PDR)手术中的应用价值.方法 选取增殖性糖尿病视网膜病变(PDR)31例31只眼,切除中轴部玻璃体后,注入曲安奈德(TA)混悬液,染色玻璃体,高速玻切切除残留玻璃体后皮质及视网膜前膜.术后随访时重点观察患者视力、眼压、白内障形成、是否有玻璃体残留再次增殖及视网膜复位情况.结果 31例患者术中注入曲安奈德后,残留玻璃体及视网膜前膜均能清晰显示,利用高速玻切可容易切除残留玻璃体及视网膜前膜.术后随访观察3~10个月,31只眼中25只眼视力不同程度提高,11(91.7%)眼视网膜复位.结论 PDR玻璃体切除术中,应用TA染色玻璃体,可清晰显示后部残留玻璃体,同时联合应用高速玻切,提高了手术的准确性和安全性,术后并未发现TA相关并发症.

Abstract:Objective To detect the value of using Tfiamcionlone acetonide (TA) to chromovitrectomy and high-speed vitrectomy in proliferative diabetic retinopathy (PDR).Methods After cut the axial vitreous of 31 cases (31eyes) of PDR,TA was used to dye the vitreous,and then high-speed vitrectomy technique was used to remove the residual vitreous and epiretinal membrane.During the time of fellow-up,visual acuity, intraocular pressure, whether or not had residual vitreous and the rate of retina reattaclament were recorded.Resuits The residual vitreous and epiretinal membrane can be easily identified and cut with TA and high-speed vitrectomy technique.The visual acuity of 25 cases was increased and 11 eyes (91.7%) retina reattachment after follow-up period for 3 to 10 months.Conclusions Residual vitreous and epiretinal membrane can be easily identified and cut with TA and high-speed vitrectomy technique.The procedures were performed easily,surely and safely.
