阿奇霉素用于治疗十二指肠溃疡和根除幽门螺杆菌

研究阿奇霉素与甲硝唑及奥美拉唑联合应用治疗幽门螺杆菌（ＨＰ）阳性的十二指肠溃疡。方法： ５３例符合条件的病人分成 ２组，其中 ２２例口服阿奇霉素 ２５０ｍｇ，每天 １次，共 ５天，甲硝唑４００ｍｇ，奥美拉唑 ２０ｍｇ，均每天 ２次，疗程 １周。另 １组口服阿奇霉素 ５００ｍｇ，每天 １次，共 ５天，甲硝唑 ４００ｍｇ，奥美拉唑 ２０ｍｇ，每天 ２次，疗程 １周。疗程结束后两组均继服奥美拉唑 ２０ｍｇ，每天 ２次，共 １周。疗程结束后 １月复查胃镜。结果：阿奇霉素 ５００ｍｇ组ＨＰ根除率 ９０．３２％与阿奇霉素２５０ｍｇ组ＨＰ根除率６８．１８％相比有显著性差异（Ｐ＜０．０５）。而两组溃疡愈合率分别为８７．１％和７２．７％，差异无显著性（Ｐ＞ ０．０５）。两组病人副反应总发生率为 １５．０９％，未发现严重副反应。结论：阿奇霉素与甲哨唑及奥美拉唑联合短程疗法有较理想的溃疡愈合率及ＨＰ根除率。阿奇霉素用量以每日 ５００ｍｇ为佳。

Clinical study of azithromycin combination therapy for the treatment of H. pylori infection in duodenal ulcer

Objectives: The efficacy of Azithromycin combination therapy in optimal dosage for the treatment of H. pylori infection in duodenal ulcer. Methods: Fifty-three patients with postive H. pylon active duodenal ulcer were recruited. Patients were randomised into two groups: Group A received omeprazole 20 mg. Metronidazole 400mg, each twice daily, Azithromycin 250mg, each one daily. group B received Omeprazole 20mg, metronidazole 400mg, each twice daily, Azithromycin 500mg, each one daily. In two groups, Azithromycin was administered for five days, mertonidazole for seven days, Omeprazole for fourteen days. All patients were determinated by endoscopy in four weeks after cession of all treatment. Results: Eradication rates of H. pylon were.68. 18% and 90. 32 % in group A and group B respectively (P 0. 05). Total side effect was 15. 09% in two groups. Conclusions: Azithromycin combined with Omeprazole and mertonidazole was highly effective in the cure of H. pylon and 500mg Azithramycin one daily gave even better result.
