原发性胃癌卵巢转移27例临床分析

目的　分析胃癌继发的转移性卵巢癌的临床特点、治疗及预后因素。方法　对 2 7例原发性胃癌卵巢转移患者的临床特征、治疗生存情况及预后因素进行回顾性分析。运用SPSS 10 0软件包分析影响预后的因素。结果　2 7例患者临床症状以腹痛和腹部肿块常见 (5 5 5 % ,15 / 2 7) ,转移性卵巢癌的病理类型以印戒细胞癌为主 (占5 9 3% ,16 / 2 7) ,双侧卵巢转移最常见 (74 1% ,2 0 / 2 7)。治疗采用手术联合化疗为主的综合治疗。全组患者中位生存期为 12 8个月 ,1年生存率为 5 7% ,2年生存率为 6 %。结论　胃癌来源的转移性卵巢癌临床病期较晚 ,综合治疗有延长患者生存期的趋势

Analysis of Metastasis to Ovary from Primary Gastric Cancer

Objective The purpose of this study was to evaluate the characteristics and the treatments of metastasis to ovary in gastric cancer and to determine prognostic factors in survival.Methods Clinical characteristics and treatments of patients with history of ovary metastasis from primary gastric cancer were analyzed with SPSS 10.0 statistical software and the probable prognostic factors for survival were vealuated as well.Results A total of 27 cases of metastasis to ovary from gastric cancer were reviewed.Abdominal pain and pelivic mass were most common in initial clinical presentations,accounting for 55.5%(15/27).Signet-ring cell carcinoma of three observed pathological types was predominant in ovary metastasis,accounting for 59.3%(16/27).Bilateral metastasis was found in 20 patients(74.1%).All patients underwent surgical treatment and most of them were fulfilled with chemotherapy or radiotherapy or both.Median survival time of 12.8 months was observed and about 57% of patients survived 1 year and less than 6% survived 2 years.Conclusion Ovary Metastasis from primary gastric cancer have late extend in disease,thus only applying multidisciplinary treatment to acquire relative longer median survival time.
