急性颅脑外伤垂体前叶激素的动态变化及其临床意义

摘    要:对24例开颅手术的急性颅脑外伤患者的血清卵泡刺激素、黄体生成激素、促甲状腺激素、三碘甲状腺原氨酸、甲状腺素、生长激素和催乳素在伤后10d内作了动态观察。结果显示,脑外伤后垂体和甲状腺功能有明显变化,变化程度与颅脑外伤的严重程度有关。我们认为,测定颅脑外伤后垂体和甲状腺功能,并作动态观察,有助于判断脑外伤程度和估计预后。

Clinical significance of dynamical changes in anterior pituitary hormones after acute head injury

Abstract:The dynamical levels of serum follicular-stimulating horrnone (FSH), luleinizing hormone (LH) , thyriod-stimulating hormone (TSH), triiodothyronine (T3), thyroxin (T4), growth hormone (GH),and prolactin(PRL) were studied in 24 undergoing craniotomy patients with acute head injuries in the first 10 days after trauma. The results showed that the average levels of serum FSH,LH,TSH,GH,and PRL were increased significantly within 24 hours after trauma compared with control group (P<0. 01, 0. 01, 0. 01, 0. 05, 0. 05 respectively). The average level of serum T3 was decreased remarkably compared with control group (P<0. 05),while the average level of serum T4 had no change. The average levels of serum FSH,LH,GH,and PRL were decreased remarkably and the average level of serum TSH was increased in varied degree after operation cotnpared to those before operation(P<0. 05,0. 01, 0. 01, 0.01 respectively), while there were not significant changes in the average levels of serum TSH,T3,and T4. The patients had a worse prognosis when the average levels of serum GH, LH,and PRL were decreased consistently. These findings suggested that there was a positive correlation between the variance of pituitary and thyroid function and the severity of head in juries. We consider that the dynamical measurements of anterior pituitary and hormones are useful to evaluate the severity and prognosis of the patients with head injuries.
