两种镇痛方法用于输卵管结扎术临床研究

目的探讨异丙酚联合芬太尼、阿托品及异丙酚联合芬太尼在输卵管结扎术中的麻醉效果及安全性。方法将200例自愿做输卵管结扎术的妇女随机分为A、B两组各100例,A组给予异丙酚联合芬太尼、阿托品麻醉,B组给予异丙酚联合芬太尼麻醉;比较两组麻醉镇静效果、呼吸、循环系统变化和不良反应。结果A、B两组无痛率分别为94%和92%(P>0.05);术中各项生命体征指标均有不同程度下降,B组较A组下降显著(P<0.05),术后各项生命体征指标均回升到术前水平。两组异丙酚总用量及意识恢复时间均无显著性差异(P>0.05)。B组术中25例血压下降≥术前20%,74例心率、血氧饱和度下降,经处理后回升;A组术中5例血压下降≥术前20%,而心率、血氧饱和度较平稳。结论异丙酚联合芬太尼、阿托品用于输卵管结扎术中麻醉镇痛效果与异丙酚联合芬太尼相当,但其作用时间短,苏醒快、无痛苦记忆,不良反应轻微,安全性高,是一种比较安全的、理想的镇痛方法。

The clinical study of two analgesias used in the ligation of fallopian tube

Objective To explore the anethesia effects and safety of diprivan combining with fentanyl vs.atropine plus diprivan combing with fentynyl in the ligation of falpian tube.Methods 200 women who voluntaryly received the ligation of fallopian tube were randomly divided into group A(n=100)receiving atropine plus diprivan combing with fentynyl and group B(n=100)receiving diprivan combining with fentanyl;anethesia effects,changes of respiration and recirculating system and adverse effects.Results The anodynia rates were respectively 94% in the group A and 92% in the group B(P>0.05);during operation all vital sign indexes decreased in varying degree,those of the group B did more significantly(P<0.05),and after operation all vital sign indexes returned to preoperative levels.There were no significant difference in total dosage of diprivan and consciousness recovery time between the two groups(P>0.05).During operation in the group B the BP decrease of 25 women was equal to or exceed 20% of preoperation BP,the HR and saturation of blood oxygen of 74 ones decreased and after treatment picked up;in the group A the BP decrease of 5 ones was equal to or exceed 20% of preoperation BP,and the HR and saturation of blood oxygen were equability.Conclusion Analgesia effects of diprivan combined with fentanyl and atropine were equivalent to diprivan plus fentanyl used in the ligation of fallopian tube,but the former has shorter action time,quick analepsia,no complaints remembrance,mild adverse effects and higher safety,and is a safe and ideal analgesia.
