| 摘 要： | 目的探讨乳腺癌前哨淋巴结活检对腋淋巴结转移的预测价值。方法采用色素法对72例原发性乳腺癌患者进行术中前哨淋巴结活检的临床研究。手术中前哨淋巴结作快速冰冻病理，术后常规石蜡连续切片病理对照，并观察比较前哨淋巴结与非前哨淋巴结的转移情况。结果（1）72例中70例成功找到前哨淋巴结，检出率为97．22％，总体准确率为98．60％，总假阴性率为3．70％（1／27）；其中前40例准确率为97．50％，后30例为100％；前40例假阴性率为5．88％（1／17），后30例假阴性率为0％。（2）70例中前哨淋巴结平均个数为2．99枚，其中前40例平均为3．48枚，后30例平均为2．33枚。70例患者中有27例淋巴结转移，其中仅前哨淋巴结转移者为77．78％（21／27）。（3）前哨淋巴结活检所用时间从最初的35．6min缩短到目前的6．7min即可完成。结论乳腺癌的色素法行前哨淋巴结活检，其检出率、敏感性、正确率都显示出良好的效果，可成为乳腺癌外科治疗上“个体化、微创化”的有益手段之一。开展乳腺癌前哨淋巴结活检替代腋窝清扫术必须经过30～50例的外科专业培训，保证假阴性率在5％以下。
