左炔诺孕酮宫内释放系统治疗子宫腺肌病的临床观察

目的探讨左炔诺孕酮宫内释放系统（LNG—IUS）对子宫腺肌病的临床疗效和安全性。方法选取2005年10月-2008年3月在温州市计划生育指导站诊治的子宫腺肌病患者55例，于月经周期第4～7天放置LNG—IUS，采用月经失血图（PBAC）及划线记录法（VAS）比较放置前及放置后3、6、12个月经量及疼痛评分，同时在放置后3、6、12个月予B超测量子宫体积及子宫内膜厚度，并观察不良反应。结果（1）患者于置环前及置环后3-．6、12个月，经量PBAC评分分别为（1954-57）分、（45±11）分、（28±13）分、（22±13）分，置环后3、6、12个月经量均较置环前明显减少，差异有统计学意义（P〈0．05）。（2）置环前患者痛经VAS评分为（73±13）分，放置3、6、12个月后VAS评分分别为（32±13）分、（18±7）分、（16±5）分，与置环前比较均显著下降，差异有统计学意义（P〈0．05）。（3）置环前平均子宫内膜厚度为（8.3±1．5）mm，置环后3．6、12个月子宫内膜厚度分别为（7．0±0．6）ram、（5．1±0．8）ram、（4．5±0．5）mm，较置环前均明显变薄，差异有统计学意义（P〈0．05）。（4）6例患者放置后出现卵巢生理性囊肿，1～6个月B超复查均自行消失。结论LNG—IUS治疗子宫腺肌病可有效缓解痛经及减少经量，临床不良反应少，是一种较为有效且安全的方法。
