肥胖、初发2型糖尿病与血清铁蛋白的相关性研究

目的探讨血清铁蛋白（SF）与肥胖、初发2型糖尿病的相关性，并探讨SF增高是否与胰岛素抵抗具有关联性。方法按体重指数（BMI）≥25kg／m^2将73例口服葡萄糖耐量试验正常者和86例初发2型糖尿病患者（T2DM）分为正常糖调节／正常体重组、正常糖调节超重／肥胖组、T2DM正常体重组、T2DM超重／肥胖组等4个亚组。测定所有研究对象的SF、空腹胰岛素、空腹血糖、糖化血红蛋白、血脂、尿酸及血压水平，并计算BMI，腰臀围比及胰岛素抵抗指数。结果肥胖、初发2型糖尿病具有较高的SF水平，与正常糖调节，正常体重组相比，有显著性差异（P〈0．05）；SF与BMI、腰臀围比、收缩压，空腹胰岛素、空腹血糖，胰岛素抵抗指数、甘油三酯及尿酸呈正相关，且SF与胰岛素抵抗指数存在一定剂量关系。结论肥胖、初发T2DM患者体内铁超负荷，而铁超负荷可能导致胰岛素抵抗。

Study on the Correlation between Obesity,Newly Diagnosed Type 2 Diabetes Mellitus with Serum Ferrtin

Objective To study the correlation between serum ferritin（SF） and newly diagnosed type 2 diabetes mellitus with obesi- ty,and to evaluate the relevance of increased serum ferritin and insulin resistance. Methods According to BMI ≥25kg/m^2,73 patients with normal oral glucose tolerance test and 86 newly diagnosed type 2 diabetes mellitus （T2DM） were divided into four subgroups.The SF,fasting insulin,fasting blood glucose, glycosylated hemoglobin,serum lipids,serum uric acid and blood pressure were measured and BMI,waist-hip ratio and insulin resistance index were calculated respectively. Results The newly diagnosed type 2 diabetes mellitus with obesity had higher serum ferritin levels than the normal glycoregulation normal weight group. There was a positive correlation between SF and BMI,waist-hip ratio,systolic blood pressure,fasting insulin,fasting blood glucose,insulin resistance index,serum triglyceride and serum uric acid levels respectively.Otherwise,there was a certain dose relationship of the SF with insulin resistance index. Conclusion This study showed that the patients of newly diagnosed type 2 diabetes mellitus obesity,had iron overload of the body, while iron overload may lead to insulin resistance.
