早产儿脐血中血小板生成调控因子的测定及意义

摘    要:目的:探讨血小板生成调控因子对早产儿血小板计数的影响。方法:我们测定了33例早产儿脐血血小板计数,并用放射免疫法(ELISA)检测了早产儿脐血中血小板生成调控因子,包括血小板生成素(TPO)、白细胞介素6(IL-6)、白细胞介素3(IL-3)、白细胞介素1β(IL-1β)的水平,并与33例正常新生儿比较。结果:与正常新生儿比较,早产儿脐血血小板计数显著低于足月组(早产儿对足月儿:167.4±55.6×109/L对366.7±49.1×109/L,P<0.01;);早产儿脐血TPO、IL-6、IL-1β水平均显著性高于足月组,分别为(早产儿对足月儿,TPO:118.9±78.4pg/ml对63.9±42.8pg/ml,P=0.04;IL-6∶55.5±49.3pg/ml对2.2±2.4pg/ml,P=0.001;IL-1β∶4.4±3.7pg/ml对1.6±2.7pg/ml,P=0.04);而两组血浆IL-3水平无显著性差异(早产儿对足月儿:25.1±10.5mg/ml对22.7±13.4pg/ml,P>0.05)。结论:早产儿娩出时同样存在血小板-细胞因子的反馈调节机制,血小板计数降低反馈性引起脐血中TPO、IL-6、IL-1β水平升高。

Abstract:Objective:To investigate the regulatory role of haematopoietic cytokine in preterm infants.Methods: The cord blood plasma levels of platelet count, haematopoietic growth factors, including thrombopoietin(TPO), interleukin 6 (IL-6), interleukin 3 (IL-3), interleukin 1β (IL-1β) were assessed in 33 premature and 33 mature cord bloods using commercial enzyme-linked immunosorbent assay (ELISA).Results:The result shows when compared to the mature infants, the platelet count in premature cord blood decreased significantly (167.4±55.6)×10 9/L vs (366.7±49.1)×10 9/L,P<0.01, while TPO , IL-6, IL-1β increased significantly (TPO: (118.9±78.4 pg/ml vs 63.9±42.8 pg/ml,P=0.04;IL-6: 55.5±49.3 pg/ml vs 2.2±2.4 pg/ml,P=0.001; IL-1β∶4.4±3.7 pg/ml vs 1.6±2.7 pg/ml,P=0.04。) No significance was observed in IL-3 level (25.1±10.5 mg/ml vs 22.7±13.4 pg/ml, P>0.05).Conclusion:We conclude that there is feedback control system exists in preterm infants as adults.
