弥漫性泛细支气管炎探讨-附24例报告

目的：提高对弥漫性泛细支气管炎（DPB）临床特点的认识。方法：对24例DPB进行回顾性分析。结果：所有患者均有咳嗽，咳痰和活动后气促三大症状，合并有鼻窦炎或既往有鼻窦炎和鼻息肉摘除史100％，本组患者人体白细胞抗原（HLA）B54阳性率53．8％，冷凝集试验＞1：64占87．5％，动脉血氧分压（PaO2)＜80mmHg占91．3％，FEV1／FVC＜70％占95．2％，胸部X线显示两肺弥漫性小结节阴影伴肺过度膨胀占60．8％，胸部HRCT显示双中下肺野弥漫性小叶中心性结节影占86．9％，病理组织学确诊19例，临床诊断5例。结论：识别DPB的临床特点，结合特征性胸部影像学表现，有助于本病的早期诊断，小剂量红霉素治疗有肯定疗效。

Diffuse Panbronchiolitis - a Clinical Report of 24 Cases

Objective To raise the cognitive level about the clinical characteristics of diffuse panbronchiolitis (DPB).Methods The data about 24 cases with DPB were reviwed and studied. Results All of the 24 cases with DPB showed three the main symptom,included cough,expectoration and polypnea after physical activities,and the complication was nasosinusitis, nasal polyps or it was excised. The results of the laboratory examination showed the that human leucocyte antigen(HLA)B 54 , positive rate was 53 8% and the cold agglutination test >1:64 positive ratio was 87 5%,and 91 3% in partial pressure of arterial oxygen (PaO 2)<80mmHg, and 95 2% in FEV 1/FVC<70% in these cases. And the chest X-ray manifestation showed that 60 8% existed with diffuse nodes shadow and hyperdistension in two lungs. And the HRCT manifestation in chest showed that 86.9% had diffuse nodes and lobule center shadow in the middle and below leafs of two lungs fields. And 19 cases were diagnosed with pathological methods, 5 ones with clinical methods. Conclusions The characteristics of the clinical situation and the imaging manifestation in chest of DPB are important for early diagnosis,main treatment method is the small dosage of erythromicy,which the curative effect was confirmed.
